摘要
目的探讨白内障超声乳化术治疗闭角型青光眼的临床疗效,评价治疗安全性。方法回顾性分析该院2010—2012年间收治的83例(89眼)闭角型青光眼患者的临床资料,所有患者均行白内障超声乳化术治疗,术后对患者进行3～6个月的随访,观察手术前后患者的视力、眼压、前房深度、视野情况以及术后并发症情况。结果该组83例患者经白内障超声乳化术治疗后视力水平较治疗前有显著提高,眼压叫治疗前有显著降低,前房深度较治疗前显著加深,差异有统计学意义(P<0.05),经静态前房角镜检查发现所有患者巩膜突可见范围均有不同程度增宽,均>180°范围,视野范围较治疗前有明显提高。该组83例闭角型青光眼患者,术后均未出现前房出血、虹膜损伤、继发恶性青光眼、视网膜脱离及后囊破裂等并发症。结论采用白内障超声乳化术治疗闭角型青光眼临床疗效确切,能够显著提高患者视力,降低眼压,加深前房深度,且术后并发症较少,具有较高的安全性,值得临床广泛应用。
Objective To investigate the phacoemulsification for angle closure glaucoma clinical efficacy, evaluation of therapeutic safety. Methods a retrospective analysis of our hospital from 2010--2012 years in 83 cases （ 89 eyes） of angle-closure glaucoma in patients with clinical data, all the patients underwent phacoemulsification on postoperative treatment, patients were 3- 6 months follow-up, were observed before and after operation in patients with visual acuity, intraoeular pressure, anterior chamber depth, view and postoperative complications. Results the 83 cases of patients after phacoemulsification vision after treatment than before treatment significantly increased levels of intraocular pressure, called before treatment were significantly decreased, anterior cham- ber depth was deeper than before treatment, the differences were statistically significant （ P〈0.05 ）, the static gonioscopy was found in all patients the scleral protrusion range have varying degrees of increase wide, all 〉180°, Ken markedly improved. This group of 83 patients with angle-closure glaucoma patients, postoperative were hyphema, iris injury, secondary to malignant glaucoma, reti- nal detachment and posterior capsular rupture and other complications. Conclusion phaeoemulsification for angle closure glaucoma clinical curative effect, can improve patient acuity, IOP reduction, deepening of the anterior chamber depth, and fewer postopera- tive complications, has high safety, is worth in clinical application.
