家庭心理干预对首发精神分裂症患者服药依从性的3年随访对照研究 被引量：10

摘要 目的:比较抗精神病药结合家庭心理干预与单用对首发精神分裂症患者服药依从性的影响。方法:采用前瞻性队列研究,将120例首发精神分裂症患者随机分为对照组(单用药物治疗组)和干预组(药物结合家庭心理干预组)各60例,随访3年。家庭心理干预包括健康教育、家庭干预、认知行为治疗及技能训练,主要评价指标为服药依从率、疾病复发率和再入院率,次要指标为自知力与治疗态度问卷(ITAQ)、PANSS快速评分表、社会功能缺陷筛选量表(SDSS)评分的变化。结果:(1)干预组和对照组服药依从率分别为58.3%和26.7%(x2=12.31);疾病复发率分别为31.6%和63.3%(x2=12.06);再入院率分别为21.7%和48.3%(x2=9.377),差异均有统计学意义(P均<0.05)。(2)干预组ITAQ总分变化值(5.1±0.2)分;PANSS总分变化值(6.3±0.4)分;SDSS总分变化值(1.1±0.1)分,对照组分别为(1.6±0.1)分、(5.7±0.2)分、(0.9±0.1)分,差异均有统计学意义(P均<0.05)。结论:抗精神病药物结合家庭心理干预治疗首发精神分裂症能提高患者的自知力和服药依从性、预防疾病复发、减少再入院率、改善生活质量。
