摘要
目的 :探讨子宫内膜癌中 p2 7Kipl蛋白的表达情况及临床病理意义。方法 :采用免疫组化S P法检测 10 5例子宫内膜癌p2 7Kip1蛋白的表达 ,并运用流式细胞术检测 46例子宫内膜癌组织的DNA含量。结果 :子宫内膜癌 p2 7Kip1蛋白阳性表达率为6 9 % (72 / 10 5例 ) ,明显低于正常增生期子宫内膜及单纯增生、复合增生子宫内膜总的阳性率 (91% ,P <0 0 1) ,而与不典型增生子宫内膜的 p2 7Kip1蛋白表达无明显差异 (P >0 0 5 )。p2 7Kip1蛋白阳性表达与子宫内膜癌的组织学分级、临床分期及DNA倍体有关 (P <0 0 5 )。组织分化差、临床期别晚以及异倍体细胞中 ,其 p2 7Kip1蛋白阳性表达明显下降。p2 7Kip1蛋白表达与子宫内膜癌组织学类型、肌层浸润、患者年龄及淋巴结转移无明显相关性。结论 :p2 7Kip1蛋白表达的下降可能与子宫内膜癌的发生发展有关 。
Purpose To investigate the expression of p27 Kip1 protein in endometrial carcinoma and its relationship with clinicopathological features and prognosis. Methods Immunohistochemical technique was used to detect the expression of p27 Kip1 in 105 cases of endometrial carcinoma.DNA ploidy was measured by flow cytometry in forty six cases. Results The rate of p27 Kip1 protein positive expression in endometrial carcinoma was 69%(72/105). It was lower significantly than the average rate of p27 Kip1 protein positive expression in normal proliferative tissue, endometrial simple hyperplasia and endometrial complex hyperplasia (91%, P <0 01). But the rates of p27 Kip1 protein positive expression between endometrial carcinoma and endometrial atypical hyperplasia have no difference significantly( P >0 05). Decrease of p27 Kip1 positive cells was correlated with higher histological grade,advanced stage and the accumulation of aneuploid cells( P <0 05). It was not related to the tissue type,depth of myometrium invasion,patients age and lymph node metastasis. Conclusion The reduction of p27 Kip1 protein expression may play an important role in the genesis and development of some endometrial carcinoma, and may be a useful prognostic index. Further studies should verify the possible significance of p27 Kip1 expression in the prognostic characterization of patients with endometrial cancer.
