摘要
目的 探索栓塞疗法治疗脑动脉瘤达到理想结果的条件。方法 1996年 4月至 2 0 0 0年 4月间栓塞治疗 3 3例动脉瘤患者。以球囊栓塞 4例 ,以机械解脱式钨丝弹簧圈栓塞 2 0例 ,以电解脱式铂金弹簧圈栓塞 9例。结果 闭塞载瘤动脉 4例 ,10 0 %闭塞 8例 ,95 %闭塞 15例 ,90 %闭塞 6例。无手术并发症。随访 (门诊或信访 ) 3 3例无再出血。造影随访 8例 ,其中术后 1个月复查造影的 2例均保持术后栓塞程度 ,术后 3个月复查的 3例中 2例瘤颈部残腔略有扩大 ,术后 6个月复查的 3例中 1例瘤颈部残腔略有扩大。结论 完全或大部分栓塞动脉瘤近期内可有效预防动脉瘤出血。但完全地、紧密地填塞动脉瘤才能获得长期稳定的栓塞结果 ,这是脑动脉瘤栓塞疗法完美实施的基础。
Objective To clarify the ideal status for treating cerebral aneurysms.Methods 33 cases with cerebral aneurysms embolized via endovascular route between April 1996 and April 2000 were analyzed. Among 33 aneurysms, 4 were balloon embolized (1 giant aneurysm, 3 dissecting aneurysms), 20 MDS (mechanical detachable sprial) embolized, 9 GDC (guglielmi detachable coil) embolized.Results\ Parent arteries occluded in 4 cases, 100% aneurysm occlusion occurred in 8 cases, 95% aneurysm occlusion in 15 cases, 90% aneurysm occlusion in 6 cases. No rebleeding was found during follow up. Angiography was carried out in 8 cases 1 to 6 months after embolization.Recurrence occurred in 3 cases. Conclusion Endovascular embolization of cerebral aneurysms is effective in preventing rebleeding. However, to obtain long term stable embolization, compact packing of aneurysm sac is needed.
