摘要
目的研究右美托咪定对高血压患者双腔气管插管应激反应的影响。方法40例择期肺部手术需双腔气管插管的高血压患者，ASA1I一Ⅲ级，年龄50。65岁，随机分为对照组和右美托咪定组，每组20例。右美托咪定组麻醉诱导前给予右美托咪定g·kg-1，溶于氯化钠注射液20mL，10min泵完，对照组泵入等容量氯化钠注射液，其余麻醉方法相同。观察并记录麻醉诱导前（To）、气管插管成功即刻（T，），插管后1min（T：）、插管后5rain（T，）和插管后10min（I＂4）患者的平均动脉压（MAP）、心率（HR）、血浆肾上腺素（E）和去甲肾上腺素（NE）的浓度，并观察不良反应。结果与T。时相比，对照组T，一T4时MAP、HR及血浆E、NE浓度均增高（P〈0．05），右美托咪定组T：时血浆E、NE浓度升高（P〈0．05），T。时降至麻醉前水平，MAP、HR各时点均无明显变化（P〉0．05）。右美托咪定组各时点MAP、HR及血浆E、NE浓度均低于对照组（P〈0．05）。两组未见明显不良反应。结论右美托咪定可以很好地抑制高血压患者双腔气管插管的应激反应，患者血流动力学稳定。
AIM To study the effect of dexmedetomidine on the stress reaction caused by double-lumen endotracheal intubation in hypertension patients. METHODS Forty ASA 11 - m with hypertension scheduled general anesthesia with double-lumen endotracheal intubation, aged 50 - 65 years old, were randomly divided into two groups （n = 20 each） . In the dexmedetomidine group, 10 min before anesthesia induction, dexmede- tomidine 1 Ixg.kg-1 was infused over 10 min. The equal volume of normal saline was given in the control group. The mean arterial pressure （MAP） , heart rate （HR） , the plasma concentration of epinephrine （E） and norepinephrine （NE） were detected and recorded before anesthesia （To）, intubation succeed （TI）, 1 min after intubation （T2）, 5 min after intubation （T3） and 10 rain after intubation （T4）. The adverse reactions were also observed. RESUI,TS Compared with those at To, MAP, HR and the concentration of E, NE at T, - T4 in the control group were increased （P 〈 0.05） . And the concentration of E, NE at T2 in the dexmedetomidine group were higher those at To （P 〈 0.05） . Compared with those in the control group, MAP, HR and the concentration of E, NE in the dexmedetomidine group were decreased at T1 - T4 （P 〈 0.05） . NO obvious adverse reactions occurred in both groups. CONCLUSION Dexmedetomidine can inhibit the stress reaction caused by double-lumen endotracheal intubation effectively and maintain hemodynamics stabilize in hypertension patients.
