摘要
目的:探讨耳鸣习服疗法(TRT)治疗耳鸣的效果。方法:对108例耳鸣患者进行TRT,内容包括声治疗和心理咨询,分别于治疗前、治疗1个月以及治疗3个月后填写耳鸣残疾评估量表(THI)和烦躁量表,根据THI量表、烦躁量表得分变化评估疗效。结果:将耳鸣残疾度量化2级及以下的患者合计为轻度,治疗前为13例(12%,13/108),治疗1个月为36例(33%,36/108),治疗3个月为62例(56%,62/108),3级及以上患者合计为重度,治疗前为95例(87%,95/108),治疗1个月为72例(58%,72/108),治疗3个月为46例(42%,46/108),治疗前和治疗后1个月后轻度和重度患者的构成比差异具有统计学意义(χ2=13.96,P<0.01)。治疗3个月和治疗1个月轻度和重度患者的构成比差异具有统计学意义(χ2=12.63,P<0.01)。将耳鸣烦躁级量化表小于二级的患者合计为轻度,治疗前,1个月,3个月分别为10例(9.3%,10/108),39例(36%,39/108),62例(57%,62/108),二级及以上患者合计为重度,治疗前,1个月,3个月分别为98例(91%,98/108),69例(64%,69/108)和46例(42%,46/108)。治疗前后烦躁程度轻度和重度患者的构成比治疗前和治疗1月,轻度和重度患者的构成比差异具有统计学意义(χ2=22.20,P<0.01)。治疗1月和治疗3个月轻度和重度患者的构成比差异具有统计学意义(χ2=9.84,P<0.01)。结论:耳鸣习服疗法治疗耳鸣能够取得较好疗效,并且效果稳定,值得临床推广。
Objective. To investigate the effect of tinnitus retraining therapy （TRT） in tinnitus manage ment. Methods. A total of 108 patients with subjective tinnitus （disease duration greater than 3 months） were recruited in this study. Tinnitus Handicap Inventory （THI） and Restless Inventory were filled out before TRT, 1 and 3 months after the therapy to evaluate the effects. Results. The tinnitus handicap degree 2 and/or less, and Restless degree 2 and/or less were evaluated as mild tinnitus, while tinnitus handicap degree 3 and/or more, and Restless degree 2 and/or more were evaluated as severe tinnitus. According to the tinnitus handicap evaluation, there were respectively 13 （13/108, 12%）, 36 （36/108, 33%） and 62 mild tinnitus cases （62/108, 56%） before treatment, after 1 month of treatment and after 3 months of treatment, respectively, while the severe tinnitus case number were 95 （95/108, 79% ter 1 month of treatment, and 46 （46/108, 42%） after before treatment, 72 （72/108, 58%） af 3 months of treatment, respectively, with significant difference （P〈0.01）. According to the Restless inventory evaluation, there were respectively 10 （10/108, 9.3%）, 39 （39/108, 36%） and 62 mild tinnitus cases （62/108, 56%） before treatment, after 1 month of treatment and after 3 months of treatment, respectively, while the severe tinnitus case number were 98 （98/108, 91%） before treatment, 69 after 1 month of treatment （69/108, 64%）, and 46 after 3 months of treatment （46/108, 42%）, respectively, with significant difference （P〈0.01）. Conclusion： TRT is effective in the treatment of tinnitus.
