摘要
为了解基因重组人生长激素(r-hGH) 和小剂量甲状腺激素对Turner 综合征(Turner syndrome, TS)女孩矮身材的促生长效果, 作者对8例年龄8.5~14.5岁(12.3±2.1岁)、骨龄8.3~12.7岁(9.8±1.7岁)TS女孩,用 r-hGH 1.0 IU/(kg·wk)和甲状腺片15mg/d进行治疗,观察6~36个月,并以治疗前的身高 增 长速率作为自身对照。结果显示:治疗后该8例TS女孩身高增长速率明显高于治疗前(2.2± 1.1cm/年),治疗最初6个月为7.9±1.6cm/ 年,第一年为7.5±1.2cm/年,第二年为6.9±0.2cm/年,第三年为5.0±0.2cm/年,而骨龄 增长速率为0.43~0.8岁/年(0.6±0.2岁/年)。由此表明,r-hGH与小剂量甲状腺激素对T S女孩有明显的促生长效果。
This study was aimed to examine the growth-promoting effects of recombi nant human growth hormone (r-hGH) and low dose of thyroid hormone on short stat u re in girls with Turner syndrome. 8 girls with Turner syndrome, at the chronolog ical age of 8.5~14.5 years(12.3±2.1 years) and bone age of 8.3~12.7 years (9. 8±1.7 years) were observed. The maximum serum GH levels in 3 patients out of 5 w ith provocative testing available were ≥10 ng/ml (10.0, 10.0 and 15.52 ng/ml, r e spectively), and in the other 2 of the 5 patients were <10 ng/ml (9.3 and 9.5 n g/ml). Thyroid function including T3, T4 and TSH were measured in 5 patients . Th e TSH in one patient was slightly higher; the T3, T4 and TSH in other patien ts were normal. All patients were treated with r-hGH 1.0 IU/ (kg·wk) and thyroide um 15 mg/d for 6~36 months. The results showed that the growth rate after treatm ent was remarkably higher than that before treatment (2.2*1.1 cm/yr). The growt h rates in the first 6 month, the first year, the second year and the third ye ar were 7.9*1.6 cm/yr, 7.5*1.2 cm/yr, 6.9*0.2cm/yr and 5.0*0.2 cm/yr, respec tively. Height standard deviation score (HSDS) increased 1.3*1.1 SD, compared w ith that pretreatment. The annual bone age growth rate after treatment was 0.6* 0.2 yr/yr(0.343~0.8 yr/yr). These data indicated that r-hGH and low dose of th yroid hormone had remarkable effects of growth-promoting in the girls with Turn er syndrome.
