摘要
目的探讨阑尾原发肿瘤的CT表现。方法对23例经手术病理证实的原发性阑尾肿瘤的CT表现进行回顾性分析,男10例,女13例。临床呈急、慢性阑尾炎表现者14例,体检发现4例,肿瘤指标升高者5例。黏液囊腺瘤9例,黏液囊腺癌7例,结肠型腺癌和类癌各3例,平滑肌瘤1例。结果 23例原发性阑尾肿瘤CT上表现为下腹部肿块20例(囊性16例,实性4例),呈类圆形或长条状,其中8例累及回盲部,12例淋巴结肿大;囊性肿块中13例病灶内可见线样分隔,8例病灶囊壁钙化,7例囊壁不规则增厚,可见壁结节;5例发生腹腔假性黏液瘤;3例仅表现为阑尾肿大而无肿块,类似急慢性阑尾炎,病理均为阑尾类癌。结论阑尾原发肿瘤的CT表现多样,可根据肿块的密度及强化表现作出初步诊断。
Objective To investigate the CT findings of primary appendiceal neoplasms（PAN）.Methods CT findings and clinical treatment in 23 patients with PAN proved surgically and pathologically were analyzed retrospectively.There were 10 males and 13 females.in the 14 cases of acute and chronic appendicitis manifestation,physical examination found in 4 cases,elevated tumor markers in 5 cases.The pathologic patterns of PAN in 23 patients included mucinous adenoma（n=9）,mucinous adenocarcinoma（n=7）,coronic type adenocarinoma（n=3）,carcinoid tumor（n=3）,and leiomyoma（n=1）.Results CT findings of 20 cases were demonstrated oval,segmented tumor in the lower quadrant（16 cases revealed cystic,Solid 4 cases）,8 cases involving the ileocecal junction,12 patients with lymph node swelling;3 cases showed only swollen appendix without tumor.In all cystic masses,13 cases of line-like separated,8 cases of lesion cystic wall calcification,7 cases of wall thickened irregularly with cystic wall nodule;and 5 cases of pseudomyxoma peritonei.3 cases showed only swollen appendix without tumor,similar to the acute or chronic appendicitis,pathology were appendiceal carcinoid.Conclusion CT performance is diversiform of PAN,and according to the tumor density and enhanced manifestation,it can make the initial diagnosis.
