摘要
目的探讨关节镜下不同手术方式治疗膝关节盘状半月板损伤的临床效果,为临床选择术式提供参考。方法对93例平均随访33个月资料完整的膝关节盘状半月板患者进行回顾性分析。48例完全型盘状半月板患者分为(A、B)2组,A组26例患者行完全切除术,B组22例患者行部分切除成形术。45例不完全型盘状半月板患者分为(C、D)2组,C组21例患者行完全切除术,D组24例患者行部分切除成形术。通过Ikeuchi氏膝关节评价和Lysholm膝关节评分标准分析手术效果。结果临床结果按Ikeuchi氏膝关节评价方法,优46例,良35例,可8例,差4例,优良率为87.1%。48例完全型盘状半月板中,26例完全切除术患者的优良率为92.3%,22部分切除成形术优良率为72.7%,依据Lysholm膝关节评分标准进行统计分析,二者差异有统计学意义(P<0.05);45例不完全型盘状半月板中,21例完全切除术患者的优良率为90.5%,24例部分切除成形术优良率为91.7%,两者差异无统计学意义(P>0.05)。结论(1)关节镜下盘状半月板完全切除术和部分切除成形术均可获得满意的疗效;(2)不完全型盘状半月板中完全切除术与部分切除成形术取得相近疗效,而完全型盘状半月板中完全切除术优于部分切除成形术。
Objective To explore the curative effects of arthroscopic treatment of discoid meniscus injuries of the knee joints in different operative methods, and to provide references for the clinical choice of operative methods. Methods The complete data of 93 patients with discoid meniscus of the knee joints who were followed up for a mean period of 33 months were retrospectively analyzed. 48 cases of complete type of discoid meniscus were divided into 2 groups. Group A （ 26 cases ） was treated with total meniscectomy, and group B （ 22 cases ） was treated with partial meniscectomy. 45 cases of incomplete type of discoid meniscus were divided into 2 groups. Group C （ 21 cases ） was treated with total meniscectomy, and group D （ 24 cases ） was treated with partial meniscectomy. The surgical results were analyzed according to the Ikeuehi＇s criteria of the knee joints and Lysholm knee score scale. Results Based on the scale of Ikeuehi＇s criteria of the knee joints, 46 eases were excellent, 35 cases good, 8 cases fair and 4 cases poor, and the excellent and good rate was 87.1%. In the 48 cases of complete type of discoid meniscus, the excellent and good rate in the 26 patients who were treated with total meniscectomy was 92.3%, which was 72.7% in the 22 patients who were treated with partial meniscectomy. The differences between the total and partial meniscectomy were statistically significant according to the Lysholm knee score scale （ P 〈 0.05 ）. In the 45 cases of incomplete type of discoid meniscus, the excellent and good rate in the 21 patients who were treated with total meniscectomy was 90.5%, which was 91.7% in the 24 patients who were treated with partial meniscectomy. There were no statistically significant differences between the total and partial meniscectomy （ P 〉 0.05 ）. Conclusions （ 1 ） Satisfactory curative effects can be achieved in the treatment of discoid meniscus injuries with total and partial meniscectomy under arthroscopy; （ 2 ） Similar curative effects can be achieved in total and partial meniscectomy for the incomplete type of discoid meniscus. However, the total meniscectomy is superior to the partial meniscectomy for the complete type of discoid meniscus.
