摘要
目的探讨优质护理服务模式指导下的护理干预措施对并发呼吸机相关性肺炎的重型颅脑损伤患者的影响。方法根据有无开展优质护理服务活动将2010年6月-2012年7月在浙江省温岭市中医院住院治疗的84例并发呼吸机相关性肺炎的重型颅脑损伤患者分为常规护理组(38例)和护理干预组(46例)。常规护理组患者接受常规护理干预措施,而护理干预组患者接受优质护理服务理念制订的护理干预措施,比较两组患者临床疗效、患者基础护理合格率和患者家属对护理人员提供护理服务的满意度。结果护理干预组和常规护理组患者住院时间[(14.38±5.14)d比(17.59±5.35)d,t=2.797,P<0.05]、住院费用[(61 365.39±2549.38)元比(64 347.46±2478.61)元,t=5.403,P<0.05]和死亡率(28.26%比47.37%,χ2=2.797,P<0.05)比较,前者明显低于(少于)后者;护理干预组的基础护理的合格率和患者家属对护理人员提供护理服务的满意率则明显高于常规护理组的(97.83%比84.21%,χ2=5.050,P<0.05;95.65%比78.95%,χ2=5.537,P<0.05),结论优质护理服务活动明显减少了住院费用,缩短了住院时间,降低了患者的死亡率,提高了患者的基础护理合格率,改善了患者家属对护理人员提供护理服务的满意度,值得进一步推广。
Objective To investigate the influence of nursing intervention measures under the guidance of high quality nurs ing service mode in severe head injury patients complicated by ventilator associated pneumonia.Methods The clinical data of eighty-four severe head injury patients complicated by ventilator associated pneumonia who were treated in the People＇s Hospital of Wenling City from June 2010 to July 2012 were collected,they were divided into the routine nursing group（38 cases） and the nursing intervention group（46 cases） according to providing high quality nursing ser vice without,patients in the routine nursing group were given general medicine routine care for severe head injury and ventilator associated pneumonia,patients in the nursing intervention group were given nursing intervention measures under the guidance of high quality nursing service mode,the clinical efficacy,the death rate,the basic nursing quali fied rate and the satisfaction for the nursing service were evaluated.Results The length of hospital stay [（14.38±5.14）d vs（17.59±5.35）d,t=2.797,P 0.05],hospitalization expenses [（61365.39±2549.38）yuan vs（64347.46±2478.61）yuan,t = 5.403,P 0.05] and the death rate（28.26% vs 47.37%,χ2=2.797,P 0.05） in the nursing intervention group were lower than that in the routine nursing group（P 0.05）,but the basic nursing qualified rate and the satisfaction for the nursing service in the nursing intervention group were higher than that in the routine nursing group（97.83% vs 84.21%,χ2=5.050,P 0.05;95.65% vs 78.95%,χ2=5.537,P 0.05）.Conclusion The nursing intervention measures under the guidance of high quality nursing service mode in severe head injury patients complicated by ventilator asso ciated pneumonia can obviously reduce hospitalization expense,shorter length of hospital stay and reduce the death rate,it can improve the basic nursing qualified rate and the satisfaction for the nursing service,and it is worth expand ing in more departments.
