摘要
目的探讨N-末端脑钠肽前体(N-terminal pro-B-type natriuretic peptide,NT-proBNP)、肌钙蛋白T(cardiactroponinⅠ,cTnⅠ)、超敏C反应蛋白(high-sensitive C-reactive protein,hs-CRP)检测诊断慢性心力衰竭的临床价值。方法选择河南省濮阳市安阳地区医院2007年1月-2012年1月确诊的慢性心力衰竭(chronic heartfailure,CHF)患者30例,另选取同期健康体检者30例作为正常对照组,检测对照组及不同心功能分级CHF患者的NT-proBNP、cTnⅠ、hs-CRP水平,及CHF患者治疗2个月后的NT-proBNP、cTnⅠ、hs-CRP水平。结果不同心功能分级CHF患者的NT-proBNP、cTnI、hs-CRP水平均较正常对照组高,差异有统计学意义(P<0.05或P<0.01)。治疗2个月后,30例CHF患者NT-proBNP、cTnⅠ、hs-CRP水平均较治疗前明显降低,差异有统计学意义(P<0.05)。随访1年后,30例CHF患者发生心血管事件6例,发生心血管事件组的NT-proBNP、cTnⅠ、hs-CRP水平明显高于未发生心血管事件组,差异有统计学意义(P<0.05)。结论检测CHF患者NT-proBNP、cTnⅠ、hs-CRP水平,有助于CHF病情评估、治疗效果评价及预后判断。
Objective To investigate the clinical value of NT-proBNP,cTn Ⅰ,hs-CRP detection in diagnosis of chronic heart failure(CHF).Methods 30 cases of patients diagnosed CHF in Anyang Region Hospital of Puyang City from January 2007 to January 2012 were selected,and 30 cases healthy person for physical examination during the same period were selected as the normal controls.NT-proBNP,cTn Ⅰ,hs-CRP were detected in CHF patients with different heart function classifications and normal controls,and after two months treatment the NT-proBNP,cTn Ⅰ,hs-CRP levels were detected in CHF patients again.Results The levels of NT-proBNP,cTnⅠ,hs-CRP in CHF patients with different heart function classifications were higher than those of the normal controls,the differences were statistically significant(P 0.05 or P 0.01).After 2 months treatment,the levels of NT-proBNP,cTn Ⅰ,hs-CRP in patients with CHF were significantly lower than before treatment,the differences were statistically significant(P 0.05).After 1-year follow-up,among 30 patients with CHF,6 cases suffered from cardiovascular events,the levels of NT-proBNP,cTn Ⅰ,hs-CRP in the cardiovascular events group were significantly higher than non-cardiovascular event group,the differences were statistically significant(P 0.05).Conclusion The levels of NT-proBNP,cTnⅠ,hsCRP detection in patients with CHF can help to assess the disease,evaluate the treatment effect and estimate progno sis.
