Relationship Between Electrocardiogram of Acute Anterior Myocardial Infarction and Infarct-related Artery

摘要 目的探讨ST段抬高型急性前壁心肌梗塞(AAMI)心电图改变与梗死相关冠状动脉(IRA)的关系。方法对84例经冠状动脉造影(CAG)证实的ST段抬高型AAMI患者的体表心电图进行回顾性分析。结果 AAMI有I、AVL导联ST段抬高,Ⅲ、AVF下移。闭塞部位多在左前降支(LAD)近段或远段并第一对角支病变,其敏感性、特异性分别为67.6%和84.0%。AAMI伴有Ⅱ、Ⅲ、AVF导联ST段抬高者,病变血管多见于LAD近段、中段合并左回旋支(LCX)或LAD远段,其敏感性为61.5%,特异性为97.5%。结论心电图可用于初步判断ST段抬高型AAMI患者,并与动脉血管闭塞部位有相关性,对临床有重要的参考价值。 Objective To probe into the relationship of electrocardiographic changes in acute anterior myocardial infarction (AAMI) with ST-segment elevation pattern and the infarction related coronary artery. Method Surface electrocardiograms in 84 cases of patients suffering from AAMI with ST-segment elevation pattern ver ified through coronary artery angiography (CAG) were retrospectively analyzed. Results I, ST segment ele- vation of AVL lead, Ⅲ and AVF depression were found in AAMI. Infarct sites were mainly located in proxi- mal or distal segment of left anterior descending branch with lesion in the first diagonal branch, of which the sensibility and specificity were 67.6% and 84.0% respectively. For patients suffering from AAMI with Ⅱ ,Ⅲ and ST segment elevation of AVF lead, vascular lesions were mainly located in proximal segment of LAD, middle segment and left circumflex artery (LCX), or distal segment of LAD, of which the sensibility was 61.5% and the specificity was 97.5%. Conclusion Electrocardiogram can be used to preliminarily diagnose patients suffering from AAMI with ST segment elevation pattern and it is correlated with sites of arterial oc- clusion, which will be of great value for clinical reference.
