17例胎儿先天性膈疝超声征像分析 被引量：4

摘要 目的探讨胎儿先天性膈疝的超声征及其产前超声诊断价值。方法 17例胎儿先天性膈疝,其中15例引产后病理证实,2例产后经床旁X线证实,回顾性分析其超声征像。结果 17例超声均表现为胎儿心脏四腔平面心旁自腹部突向胸腔的异常回声;左膈疝14例,12例表现为大囊性伴混合性回声6例,伴小囊性暗区4例,无胃泡12例,大囊性暗区蠕动8例;右膈疝3例表现为肝实质样回声;心脏向健侧压移位17例,腹围明显小于孕周14例,羊水过多8例;伴其他脏器畸形5例。结论四腔平面有利于胎儿先天性膈疝观察;胎儿先天性膈疝的主要超声表现为左侧心旁大囊性、小囊性暗区,膈下无胃泡,右侧心旁肝实质样回声;大囊性暗区蠕动征像为胎儿先天性膈疝的特征性表现;伴随的其他征像和畸形对胎儿先天性膈疝的诊断具有重要的参考价值。
