掌侧锁定加压钢板结合支垫钉技术治疗背侧不稳定性桡骨远端骨折 被引量:6

摘要 目的探讨掌侧锁定加压钢板(LCP)结合支垫钉技术治疗背侧不稳定性桡骨远端骨折的疗效。方法 2008年3月至2011年12月,采用掌侧LCP结合支垫钉技术治疗46例背侧不稳定性桡骨远端骨折。男15例,女31例;年龄36~80岁,平均58岁。根据AO骨折分型,C1型19例,C2型18例,C3型9例。根据Dienst腕关节功能评分评价术后疗效。结果所有患者术后均获12~39个月随访(平均20个月)。患者骨折均愈合,无钢板螺钉松动断裂。掌倾角由术前平均-7.15°±5.77°改善至术后平均8.12°±3.09°(P<0.01),尺偏角由术前平均9.12°±6.39°改善至术后平均21.69°±2.51°(P<0.01)。术后3个月Dienst腕关节功能评分评价显示,优27例,良16例,可3例,差0例,优良率为93.5%。结论掌侧LCP结合支垫钉技术是治疗背侧不稳定性桡骨远端骨折的有效方法,可有效恢复掌倾角并牢固维持复位,有利于患肢早期功能锻炼。 Objective To explore the efficacy of volar locking compression plate (LCP) combined with support stud technique in treatment of dorsally unstable distal radius fractures. Methods From March 2008 to December 2011, forty-six patients with dorsally unstable distal radius fracture were treated with volar LCP combined with support stud technique. There were 15 males and 31 females, aged from 36 to 80 years with the mean of 58 years. There were 19 cases for type CI, 18 cases for type (22 and 9 cases for type C3 according to AO classification criteria of fractures. The efficacy were evaluated according to Dienst wrist function score. Results All the patients were followed up form 12 to 39 months (mean 20 months) postoperativdy, and the fractures were healing, no plate or screw loosening and breakage. The volar tilt angle increased from an average of - 7. 15° ± 5.77° to 8. 12° ± 3.09° (P〈0.01), radial inclination angle increased from an average of 9. 12° ± 6. 39° to 21.69° ±2. 51 ° (P〈0. 01 ). Three months after the operation, 27 cases were graded as excellent, 16 cases as good and 3 cases as fair according to Dienst wrist function score, the overall satisfactory rate was 93.5%. Conclusions Volar LCP fixation combined with support stud technique is a safe and effective method for treating the unstable distal radius fracture of type C. It is more beneficial for replacing, recovering the volar tilt angle and early function.
