摘要
目的探讨前列腺增生手术后发生医院感染的情况及其影响因素,为降低患者发生医院感染风险提供参考。方法医院2005年1月-2012年1月共收治前列腺增生手术治疗患者507例,选取其中发生医院感染患者36例为观察组,对感染患者的临床资料进行回顾性分析,另随机选取同期未发生医院感染的前列腺增生手术患者50例为对照组,通过对比探讨前列腺增生手术后发生医院感染的情况及其影响因素。结果≤60岁患者医院感染发病率为4.5%,>60岁患者医院感染发病率为8.7%;≤60岁患者占总数的25.0%,>60岁患者占总数的75.0%;观察组的尿管留置时间为(12.1±2.5)d,明显多于对照组的(4.5±1.9)d,另外留置时间>10d患者均发生医院感染。结论应尽量减少尿管的留置时间,同时注意护理过程中的无菌操作,以降低医院感染的发生。
OBJECTIVE To investigate the status of nosocomial infections that occurred after the prostatic hyperplasia surgery and its influencing factors so as to reduce risk of the patients suffering from the infections. METHODS A total of 507 cases with benign prostatic hyperplasia, who were treated in the hospital from Jan 2005 to Jan 2012, were chosen as the research objects. 36 cases with the nosocomial infections were set as the observation group. The clinical data of infected patients were analyzed retrospectively. 50 cases of benign prostatic hyperplasia patients without nosocomial infections were chosen at the same period as the control group. The incidence of the nosocomial infections after the benign prostatic hyperplasia surgery and its influencing factors were compared and analyzed. RESULTS The incidence rate of the nosocomial infections was 4. 5% in the patient aged less than 60 years old and 8. 7% in the patients aged more than 60 years old ; the patients with less than 60 years of age accounted for 25.0%, and the patients with more than 60 years of age 75.0%. The indwelling catheter duration was（12.1±2.5） days in the observation group, significantly longer than（4.5 ± 1.9） days of the control group, and the nosocomial infections occurred in all the patients with the indwelling catheter duration more than 10 days. CONCLUSION It is necessary to shorten the retention time of the catheter, while paying attention to the aseptic operation during the nursing process , so as to reduce the incidence of nosocomial infections.
