摘要
目的分析肿瘤化疗患者发生医院感染的相关因素,并探讨防范措施,为更好地预防医院感染发生提供理论依据。方法选取768例住院化疗的肿瘤患者作为研究对象,对其中发生医院感染患者的临床资料进行回顾性分析。结果有74例肿瘤化疗患者发生医院感染,感染率为9.6%;肺部肿瘤最容易引起医院感染,占71.6%;感染部位以呼吸系统最常见,占75.7%;病原菌检测有36例为阳性,其中以革兰阴性杆菌检出率最高,占61.1%;老年人、长期住院、晚期肿瘤、化疗疗程多、白细胞抑制程度重以及合并其他并发症是肿瘤化疗患者发生医院感染的主要相关因素。结论肿瘤化疗患者医院感染发生率随着年龄增长、住院时间延长、病情进展、化疗疗程增加、白细胞抑制程度加重而增加;积极采取有效预防措施,保护易感人群,可以有效降低医院感染的发生率,值得临床推广和普及。
OBJECTIVE To analyze the related factors of nosocomial infections in the tumor patients who underwent the chemotherapy and to discuss the preventive measures so as to provide theoretical basis for prevention of nosocomial infections. METHODS A total of 768 tumor patients, who underwent the chemotherapy in the hospital, were selected as the study objects, and the clinical data of ＂the patients with the nosocomial infections were retrospectively analyzed. RESULTS The nosocomial infections occurred in 74 cases of the cancer patients with the infection rate of 9.6% ;the patients with lung cancer were prone to be infected, accounted for 75.7% ;there were 36 cases with the pathogens tested positive, and the detection rate of the gram-negative bacilli was the highest （61. 1%）. The advanced age, long hospital stay, advanced tumor, frequent chemotherapy, leucocyte inhibition degree, and other complications were the main related factors of the nosocomial infections. CONCLUSION The incidence of nosoeomial infections is increased along with the increase in the age, hospitalization duration,disease progress, chemotherapy course, and leucocyte inhibition degree. It is necessary to take effective prevention measures to protect the susceptible population so as to effectively reduce the incidence of the nosocomial infections, which is worthy of the clinical promotion.
