摘要
目的 :评价因听骨链固定、坏死或脱位而行听骨成形术的临床疗效。方法 :将残存的槌砧骨雕磨成一定形状的骨柱 ,形如倒置的洒杯 ,一端磨出髋臼关节凹 ,另一端磨出槽状关节面 ,镶嵌于镫骨头和槌骨柄之间。槌骨坏死则将砧骨长突改建成“槌骨柄”。镫上结构缺损 ,则将槌砧骨改形后连接于镫骨足板和鼓膜之间。结果 :术后 3月 ,1 71 / 1 96耳听力提高 >1 0 d B,总有效率是87.2 % ;1 4 3 / 1 96耳达到应用水平。骨气—导间距 <2 0 d B占 68% ,1年后为64 %。随访 3～ 5年仍保持稳定的听力水平。结论 :采用自体残存听骨行听骨成形术治疗慢性化脓性中耳炎。
Objective: To evaluate the curative effect of ossiculoplasty in those patients who have sustained fixtion,dislocation,or dissolution of the ossicular chain as a result of traume,infection,or congenital problem Methods:The shaped columella were sculptured in the remnants of the incus and malleus The minor columella is similar to a cup in the shape A small acetabulum has been bored out to fit the capitulum of the stapes and a notch has been made to hold out the manubrium of the malleus The long process was served as manubrium if the malleus is defective The placements remaked were created by using the malleus and incus and to rest between the footplate and tympanic membrane Results:The total effective rate of hearing gain(>10dB)was 87 2% 143 cases of 196 had regained socially acceptable hearing ability The residual air-bone gap was <20dB in 68% of case 3 months after surgery,in 64% 12 months later And then remained stable over time,even in patients seen 3～5 years after the operation Conclusion: This type of ossiculoplasty is an adeguate procedure for primary restoration of hearing in chronic otitis media,congenital malformation of the middle ear and ossicular chain dislocation
