摘要
目的观察手术体位对腹腔镜全子宫切除术患者眼内压的影响。方法选择2011年5月~2012年7月在我院择期行腹腔镜全子宫切除术患者40例。将其随机分为T组和D组,每组20例。T组取截石位,头低脚高位30°后,调高手术床背板至水平,D组作为对照组,取截石位,头低脚高位30°体位。两组麻醉方法相同。分别于全麻后5min(T0)、手术体位5min(T1)、手术体位10min(T2)、20min(T3)、30min(T4)、40min(T5)和手术结束恢复平卧位10min(T6)时,记录眼内压,同时观测SBP、DBP、HR的变化。结果术中各时点SBP、DBP、HR差异无统计学意义(P>0.05),与T0比较两组患者T1~5时点眼内压显著升高(P<0.05);T组与D组同一时间点比较,眼内压显著降低(P<0.05)。结论采用调高手术床背板至水平,改变手术体位的方法,可有效抑制腹腔镜全子宫切除术患者术中高眼内压的发生。
Objective To observe the effect of intraoeular pressure of patients on surgical position of laparoseope with total hysterectomy resection. Methods 40 cases of patients who were scheduled for laparoscope with total hysterectomy resection in our hospital during May 2011 - July 2012 were included. They were randomly divided into group T and group D, 20 cases in each group. Lithotomy position of T group taking 30 ;of trendelenburgposition, then raised the back of operation bed to level, group D as a control group, the lithotomy position took 30 ;of trendelenburgposition of position. The two groups used the same method of anesthesia, respectively after anesthesia 5rain (T0), surgical position 5 rain (T1), the operative positioning 10min (T2), 20 min (T3), 30 rain (T4), 40min (TS), and restore the supine position 10 min (T6) after the end of surgery, at that time, recording in intraocular pressure, and observing the change of SBP, DBP, HR. Results About surgery difference at each time point was not statistically significant in the SBP, DBP, HR, compared with TO the two groups of patients intraocular pressure was significantly higher with T1-5 point in time (P〈0. 05) ; compared group T and group D at the same time point. The intraoeular pressure was significantly reduced (P〈0.05). Conclusion Using the method of raised the backplane of operation bed to the level for change the surgical position can inhibit the occurrence of high intraocular pressure of laparoscope with total hysterectomy resection patients.
