摘要
目的 :研究血、尿α1 微球蛋白 (MG)在晚期肺癌化疗中对肾功能评价的临床应用价值。方法 :将 80例晚期肺癌患者随机分为 2组 ,小剂量顺铂组 40例 (DDP 6 0mg/m2 ) ,大剂量顺铂组 40例 (DDP 10 0mg/m2 ) ,分别在化疗前、化疗后 2 4— 48h和 3周时测定血、尿α1 MG及同期测得的血尿素氮 (BUN)、肌酐 (Cr)进行分析比较。结果 :化疗前、化疗后 2 4— 48h小剂量顺铂组尿α1 MG明显升高 (P <0 0 5 ) ,而大剂量顺铂组血、尿α1 MG明显升高 (P <0 0 5 ) ,3周后复查均恢复正常。BUN、Cr在化疗前、化疗后 2 4— 48h和 3周时均无明显差异 (P >0 0 5 )。结论 :联合测定血、尿α1 MG能够较早地反映轻微肾小球、肾小管功能损害 ,且较血BUN。
Purpose:To study the clinical application of blood and urine α 1 microglobin as the standard of renal function evaluation in advanced lung cancer treatment. Methods:80 cases of advanced lung cancer were devided into 2 groups evenly according to the dosage of DDP applied by chance (60?mg/m 2 and 10?mg/m 2 respectively). The blood and urine α 1 microglobin and blood BUN、Cr were analysed before and 24 48h after chemotherapy and the 3rd week from the onset of treatment. Results:Statistical significance ( P <0 05) of the urine α 1 MG was observed in the group with small cDDP dosage before and 24-48?h after chemotherapy. Statistical significance ( P <0 05) of the blood and urine α 1 MG were obviously observed in the group with large cDDP dosage before and 24-48?h after chemotherapy. However,re examination of the blood and urine α 1 MG for both groups in the 3rd week after chemotherapy showed them to be back to normal level. In comparison,the blood BUN and Cr had no apparent variation before and after the chemotherapy ( P >0 05). Conclusions:As a standard for renal function evaluation,especially for slight impairment, the blood and urine, α 1 MG are more sensitive than the blood BUN and Cr.
