腓肠神经-小隐静脉营养血管远端蒂皮瓣修复下肢远端软组织缺损

Application of distally-based flap with nutrient vessels of sural nerve and small sphenous vein in repairing soft tissue defect of distal lower extremity

摘要 目的观察腓肠神经-小隐静脉营养血管远端蒂皮瓣转移修复小腿远端软组织缺损的临床效果。方法对收治的22例小腿远端、足踝部软组织缺损患者行腓肠神经-小隐静脉营养血管远端蒂逆行岛状皮瓣转移修复术。皮瓣面积最大16.0 cm×10.0 cm,最小6.5 cm×5.5 cm,平均11.2 cm×7.4 cm;蒂长4~8 cm,蒂宽3~4 cm。结果 22例患者术后转移皮瓣血供良好、存活20例,随访6~18个月,皮瓣质地满意;出现坏死2例,但筋膜层存活,经游离皮片植皮后愈合。结论腓肠神经-小隐静脉营养血管远端蒂皮瓣切取简便,血供可靠,不牺牲主要动脉,外观满意,存活率高,是修复小腿远端软组织缺损的一种良好方法。 Objective To explore the clinical application of the distally-based flap with nutrient vessels of sural nerve and small saphenous vein in repairing the soft tissue defect of distal lower extremity.Methods 22 cases of soft tissue defect in distal lower leg,ankle and foot were transplanted by the distally-based flap with nutrient vessels of sural nerve and small saphenous vein.The areas of transplanted flaps were 16.0 cm×10.0 cm in maximum,6.5 cm×5.5 cm in minimum and 11.2 cm×7.4 cm in average.The pedicel was designed with a length of 4-8 cm and a width of 3-4 cm.Results After operation,the blood supply of transferred flap in 22 cases was all good,and the flaps survived in 20 cases with satisfactory assessment after 6-18 months follow-up.In the other 2 failed cases,the cutaneous covering of the flap was necrotic,but the fascial layer survived.By free skin grafting,raw surface healed well.Conclusion Distally-based flap with nutrient vessels of sural nerve and small saphenous vein has easy dissection,reliable blood supply,major artery preservation,satisfactory appearance and high survival,which is a better method for repairing the soft tissue defect of distal lower extremity.
