摘要
目的探讨食管癌肉瘤的组织形态学及蛋白表达特点,明确其组织来源,以提高对该病的诊断水平。方法运用苏木精-伊红(HE)染色法、胶原纤维(VG)和Masson三色特殊染色法了解食管癌肉瘤的组织形态学特点;通过免疫组化染色法了解食管癌肉瘤的蛋白表达情况。结果食管癌肉瘤常在食管中下段呈息肉样向管腔生长,由癌和肉瘤两种成分组成;特殊染色显示肉瘤成分为纤维组织;免疫组化染色显示癌区CK5/6强阳性,Ki-67阳性,而Vimentin、Desmin和S-100蛋白阴性;肉瘤区Vimentin强阳性,CK5/6呈灶性区域散在弱阳性,Ki-67弱阳性,而Desmin和S-100蛋白阴性。结论食管癌肉瘤是一种特殊的食管恶性肿瘤,由两种不同的成分组成,可能起源于食管黏膜的多潜能原始干细胞,少部分向上皮细胞方向分化形成癌,大部分向间叶细胞方向分化形成肉瘤。这为食管癌肉瘤的诊断提供了理论依据和诊断方法,同时也为食管癌肉瘤的后续治疗提供了治疗依据。
Objective To investigate the histomorphology and protein expression characteristics of esophageal carcinosarcoma for determining their histological origin and improving the diagnosis level of this disease.Methods To understand the histomorphology features of esophageal carcinosarcoma by HE staining,VG and Masson trichrome special staining,and the protein expression of esophageal carcinosarcoma by immunohistochemistry staining.Results Esophageal carcinosarcoma usually locates in the middle-lower segment of the esophagus with the polypoid growth to the lumen,which is composed by carcinoma and sarcoma two compositions.The special staining shows that the sarcoma component is fibrous tissue.The immunohistochemical staining shows that the cancer area is strong positive CK5/6,positive Ki-67,and negative Vimentin,Desmin and S-100;the sarcoma area is strong positive Vimentin,CK5/6 with scattered focal regional and weakly positive,weakly positive Ki-67,and negative Desmin and S-100.Conclusion Esophageal carcinosarcoma is a special kind of esophageal malignant tumor composed of two different components,which is probably originated from the esophageal mucosal pluripotency primitive stem cells,a few parts differentiating into epithelial cells and forming carcinoma,and most parts differentiating into mesenchymal cells and forming sarcoma,which provids the diagnostic theoretical basis and diagnostic methods for diagnosing esophageal carcinosarcoma and the treatment basis for its follow-up treatment.
