高危乳腺癌40例临床特征和预后因素分析 被引量：10

摘要 目的分析腋窝淋巴结转移≥4个的高危乳腺癌患者的临床特征和预后因素。方法回顾性分析病理证实的40例腋窝淋巴结转移≥4个的高危乳腺癌患者的临床特征和治疗结果。28例(70%)患者行术后辅助放疗。34例(85%)患者行术后辅助化疗。辅助内分泌治疗根据激素受体情况进行。2检验比较组间比例的差异,Kaplan-Meier法分析生存率,log-rank检验比较组间差异。结果全组40例高危乳腺癌患者,年龄28～70岁,中位年龄为46.5岁,年龄<40岁和≥40岁的患者分别为5例和35例。ER或PR阳性19例(47.5%),Her-2强阳性18例(45.0%)。腋窝淋巴结清扫7～54个,中位个数为18.5个,中位腋窝淋巴结阳性数为10.5个(4～40个),中位腋窝淋巴结转移率为64.1%(17.4%～100%)。全组患者中位随访时间19.3个月。2年总生存率和无进展生存率分别为65.8%和52.1%。单因素分析发现,腋窝淋巴结清扫<10个、未行放疗和化疗是预后不良因素。结论腋窝淋巴结转移≥4个的高危乳腺癌患者Her-2强阳性多见,腋窝淋巴结转移率高。腋窝淋巴结清扫个数<10个、未行放疗和化疗是预后不良因素。
