A survey on the use of antibiotics in a hospital in 2011

摘要 目的了解本院抗菌药物的使用情况,为临床合理用药提供参考。方法采用金额排序法和用药频度(DDDs)排序法对医院2011年抗菌药物应用情况进行回顾性分析。结果 2011年医院抗菌药物销售额占全部药品销售额的18.06%;抗菌药品销售金额排在前3位的药品为头孢菌素类、氟喹诺酮类、β-内酰胺及β-内酰胺抑制剂类药物,其金额构成比分别为37.04%、13.63%和12.88%;抗菌药物DDDs排在前3位的为头孢菌素类、氟喹诺酮类和硝基咪唑类;头孢菌素中,第三代头孢菌素用药金额最高,第二代头孢菌素DDDs最大。结论本院抗菌药物用药结构比较稳定,但仍存在一些隐患,应加强抗菌药物的临床应用管理,同时强化临床微生物检测与细菌耐药性监测工作。 Objective To ascertain the use of antibiotics in a hospital and provide guidance for rational use of drugs. Methods A hospital's outlays on antibiotics and defined daily doses (DDDs) of antibiotics in 2011 were determined and analyzed. Results The hospital's outlays on antibiotics accounted for 18.06% of its total drug purchases in 2011. The 3 largest outlays on antibiotics were for cephalosporins, quinolones, and β-1actams and beta-lactamase inhibitors. Cephalo- sporins accounted for 37.04% of these outlays, quinolones for 13.63%, and β-lactams and beta-lactamase inhibitors for 12.88%. The 3 largest DDDs were of cephalosporins, quinolones, and nitroimidazoles. The largest outlay on cephalospo- rins was on third-generation cephalosporins while the largest DDD was of second-generation cephalosporins. Conclusion Antibiotic use in this hospital was relatively consistent, but there are some potential pitfalls. Management of the clinical use of antibiotics must be enhanced and clinical microbiological detection must be improved along with monitoring of drug resistance.
