摘要
目的:比较丙泊酚复合芬太尼和丙泊酚复合舒芬太尼应用于人工流产对术后宫缩痛的影响,探讨人工流产手术合理的丙泊酚麻醉配方。方法:将健康、早期妊娠、要求行人工流产的孕妇100例随机分为两组(n=50),对照组(A组)丙泊酚复合芬太尼;观察组(B组)丙泊酚复合舒芬太尼。A组先缓慢静脉注射芬太尼0.6μg/kg,5min后静注丙泊酚1.5～2mg/kg开始手术,术中视体动情况和手术时间长短适当静脉追加丙泊酚20～30mg;B组于丙泊酚应用前先缓慢静脉注射舒芬太尼0.1μg/kg,余同A组。术中监测并记录血压(BP)、心率(HR)、血氧饱和度(SpO2),比较两组病人用药前后BP、HR、SpO2的变化、术中丙泊酚用量及术后发生宫缩痛的情况。结果:两组患者BP、HR、SpO2和意识消失时间比较差异无统计学意义(P>0.05);两组患者丙泊酚用量及清醒后0～30min内发生宫缩痛(采用疼痛视觉模拟评分)比较差异有统计学意义(P<0.05)。结论:两组方法用于无痛人工流产均安全有效,但B组方法对于术后宫缩痛的防治效果好,且丙泊酚用量少,故优于A组方法,是人工流产手术较为理想的丙泊酚麻醉配方。
Objective：For propofol composite fentanyl and propofol composite sufentanil used in abortion postoperative uterine contractions pain compared the effect,discusses the reasonable propofol anesthesia formula.Methods：100 patients with early pregnancy,requirements do abortion pregnant women were randomly divided into two groups（n=50）,and the control group（group A） propofol composite fentanyl;The observation group（group B） propofol composite sufentanil.A Group propofol before application slow static note fentanyl 0.6 μg/kg,5 min static note after propofol 1.5 ~ 2 mg/kg started operation,according to the needs of appropriate add with propofol 20 ~ 30 mg;Group B propofol before application slow static note of sufentanil 0.1 μg/kg,the rest of the method and A group of the same.Intraoperative monitoring and record the blood pressure and heart rate,blood oxygen saturation,compared with two groups of patients before and after use the blood pressure and heart rate,blood oxygen saturation change,operation of propofol dosage and happened after surgery uterine contraction of the pain.Results：Two groups of patients,the blood pressure and heart rate,blood oxygen saturation and unconsciousness comparative differences didn＇t statistically significant（P 0.05）;two groups of patients propofol dosage and awake 0 ~ 30 min after happening in the contractions pain（the pain visual analogue scale） comparative differences are significant（P0.05）.Conclusion：Two groups of methods are used to painless abortions are safe and effective,but group B method can control postoperative uterine contractions pain,propofol use quantity is little,the method is better than the group A method,is the artificial abortion more ideal propofol anesthesia formula.
