摘要
[目的]探讨范永升教授治疗自身免疫性肝病临床经验。[方法]从病名、病因病机、治疗原则、辨证论治、用药特点等方面论述了范永升教授治疗自身免疫性肝病临床经验。[结果]自身免疫性肝病的基本病机是肝郁血瘀。本病病变的性质是本虚标实,本虚主要为肝脾肾阴阳亏虚,标实主要为气滞、血瘀、水停。治疗原则早期疏肝理气健脾,进展期清热祛湿退黄或疏肝理气祛瘀,晚期活血化瘀利水。活血祛瘀应贯穿治疗始终。临床主要分肝郁脾虚夹瘀、肝胆湿热夹瘀、肝阴亏虚夹瘀和脾肾阳虚夹瘀四个证型。用药特点为活动期注重清热利湿和始终强调活血祛瘀。[结论]范永升教授治疗自身免疫性肝病临床经验值得推广。
[Objective]To discuss Professor Fan Yongsheng＇s experience in treating autoimmune liver disease.[Method]To relate and analyze professor Far Yongsheng＇s experience in treating autoimmune liver disease from the name of disease, etiology and pathogenesis, treatment principle, syndrome differentia tion, treatment and other aspects. [ Result]The basic pathogenesis of autoimmune liver disease is liver depression and blood stasis.The nature of the lesion i＇. weakness of origin in deficiency of Yin and Yang in liver spleen and kidney, strongness of surface in stagnation of Qi blood stasis and retention of water The therapeutic principle is carding Liver Qi and strongthening spleen Qi on early stage, clearing heat and dampness on progressive stage,and promotinf blood circulation and clearing water retenfon.The treatment of removing blood stasis should be through always. There are following main four types ir clinic： depression of liver and weakness of spleen, damp and hot in liver and gallbladder, deficiency of Yin in liver and deficiency of Yang in spleen ant kidney. The characteristics of using drag are paying attention to clearing heat and damp in active stage and always emphasizinjg removing blood stasis [Conclusion]Professor Fan Yongsheng＇s experience in treating autoimmune liver disease is worth to spread.
