A Study of MRI Multi-sequences in Spinal Metastases

摘要 目的探讨脊柱转移瘤在MRI多个序列的表现及其诊断价值。方法对经临床治疗、随访和病理证实的51例脊柱转移瘤进行T1WI、T2WI、T2WI/STIR、DWI序列的MRI扫描,研究其在MRI各序列的表现,比较各个序列的病灶检出率,对病变椎体与正常椎体在DWI上的表观弥散系数(apparent diffusion coefficient,ADC)进行统计学分析。结果 51例脊柱转移瘤受累椎体320个,胸椎最多,其次是腰椎;全椎体受累65个,38个椎体伴压缩性骨折,83个椎体附件受累,11例有椎旁软组织肿块,所有病例椎间盘未见异常。T1WI上256个椎体呈低信号,T2WI上113个椎体呈混杂信号,46个椎体呈高信号,T2WI/STIR上280个椎体呈高信号,DWI上所有椎体为高信号。T1WI、T2WI/STIR和DWI序列病灶检出率均高于T2WI,且差别有显著性意义。在b=500s/mm2时,病变椎体的ADC值高于正常椎体的ADC值,其差别具有统计学意义。结论脊柱转移瘤在MRI多个序列上有一定的特征性表现,MRI的多序列成像相互印证和补充,更能准确发现病变和判断病变的范围及程度。 Objective To explore the appearance and diagnostic value of MRI multi-se- quences in spinal metastases. Methods 5 lpatients with spinal metastases confirmed by clinical treatment, follow-up and pathology underwent MRI scans with T IWI, T2WI, T2WI/STIR and DWI sequences, MRI multi-sequences appearance and the rate of lesion detection were studied, and ADC of spinal metastases and normal vertebrae were analyzed. Results In 320 involved vertebraes of 51 cases, thoracic is the most colnmon seen and lumber is the second; the whole vertebraes involved were 65, 38 vertebraes with compres- sion fractures, 83 with vertebral appendix involved, 11 cases with para-vertebral soft tissue mass and vertebral discs of all cases were normal. 256 vertebraes showed low signal inten- sity on TIWI, 43 showed mixed signal and 46 showed high signal on T2WI, 280 showed high signal on T2WI/STIR and all showed high signal on DW1. The rates of lesion detection by TIWI, T2WI/STIR and DWI were higher than that of T2WI with significant difference. ADC of spinal metastases was higher than that of normal vertebraes with significant difference when b£1/2500s/mm2. Conclusion The spinal metastases had characteristic mani- festations on MRI multi-sequences. The mutual validation and supplement of MRI multi- sequences could more accurately detect lesion and judge the scope and degree of spinal metastases.
