老年心力衰竭患者心功能与血浆脑钠肽N末端前体、肌钙蛋白T及左心室射血分数的关系 被引量:10

The Relationship Among the Precursor of Plasma Brain Natriuretic Peptide N-terminal(NT-proBNP),Troponin and Left Ventricular Ejection Fraction in Elder Patients with Heart Failure

摘要 目的探讨老年心力衰竭(EH)患者血浆脑钠肽N末端前体(NT-proBNP)、肌钙蛋白T(cTNT)与左室射血分数(LVEF)的相关性。方法选择我院心内科80例老年心衰患者作为观察组,体检中心体检正常的80例作为对照组。检测两组患者血浆NT-proBNP、cTNT水平,运用彩色多普勒超声进行LVEF测定,并进行相关性分析。结果心衰组NT-proBNP较对照组显著升高,两组指标差异有统计学意义(P<0.01),cTNT较对照组显著升高,两组指标差异有统计学意义(P<0.05),LVEF较对照组显著降低,呈负相关,两组指标差异有统计学意义(P<0.05)。NT-proBNP在各级心功能不全之间对比,差异有统计学意义(P<0.05)。cTnT、LVEF在心功能Ⅱ~Ⅲ级与心功能Ⅳ级之间对比,差异有统计学意义(P<0.05)。结论心力衰竭患者血浆脑钠肽N末端前体和肌钙蛋白随着心力衰竭程度的加重而相应升高,左室射血分数明显减低。 Objective To investigate the precursor of plasma brain natriuretic peptide N-terminal (NT-proBNP),troponin (cTNT)and left ventricular ejection fraction (LVEF)correlation in elder patients with heart failure. Methods 80 cases of elder patients with heart failure in Department of Cardiology at our hospital were taken as the observation group,and normal 80 cases checked by the examination center at our hospital were taken as the comparison group. The plasma NT-proBNP and cTNT levels of two groups of patients were detected, and LVEF were determined by color doppler ultrasound, which were followed by the correlation analysis. Results The NT-proBNP of the heart failure group was significantly higher than that of the comparison group,in which there was a significant difference with P〈0. 01 between the two sets of index. The cTNT of the heart failure group is significantly higher than that of the comparison group, in which there was a significant difference with P〈0. 05 between the two sets of index. The LVEF of the heart failure group was significantly lower than that of the comparison group,in which negative correlation was observed and there was significant difference with P〈0. 05 between the two sets of index. For the comparison of NT proBNP among all levels of cardiac dysfunction,there is a significant difference with P〈0. 05. For the comparison of cTnT and LVEF between cardiac function Ⅱ-Ⅲ and cardiac function Ⅳ ,there is a significant difference with P〈0.05. Conclusion For the patients with heart failure, plasma brain natriuretic peptide N-terminal precursor and troponin increase with heart failure severity, and left ventricular ejection fraction is significantly reduced.
