摘要
目的 采用多功能动力复位装置治疗先天性髋脱位,对治疗后10年的患者进行随访,探讨X线结果、临床功能与治疗方式之间的关系,找出解决不同年龄、不同脱位程度患儿的治疗方法及防止并发症出现的具体措施。方法138例共158髋,复位时平均年龄为22个月,其中59髋单纯应用该装置复位固定,64髋行内收肌、骼腰肌切断后应用该装置复位成功,35髋于Ferguson术后安装动力复位装置。结果 随访时平均年龄 11.8岁,平均随访10年6个月。根据周永德评定标准,优良率为91.1%。股骨头坏死率为5.7%。结论 多功能动力复位装置的牵引、自动复位功能减少了人为复位造成的创伤,提高了闭合复位的成功率。该装置特定的外展角度及固定期间持续对抗髋周挛缩肌群的作用力,使头臼间压力减少,将股骨头坏死率控制在6%以下。在保证复位稳定的前提下,锻炼时动静结合,使关节僵硬等并发症发生率降至为零。远期疗效说明应用多功能动力复位装置是一种疗程短、效果好、痛苦少、并发症低的方法。
To review the long-term results of the treatment of congenital hip dislocation using polyfunctional reduction apparatus. Methods 138 patients (158 hips) were included in this study, the average age of the patients was 22 months. 59 hips were reduced simply by the apparatus. 64 hips were reduced and after tenotomy of adductor and iliopsoas and then the apparatus was applied. 35 hips were reduced by Ferguson's operation and then fixation using the apparatus. Results The average follow-up period was 10 years and 6 months. The excellent and good results were seen in 73. 3% radiographically and 91. 1% clinically, necrosis of the femoral head occured in 5. 7% of the cases. Conclusion The polyfunctional reduction apparatus was of help to the reduction maneuver and is able to abduct the thigh and to continueously make traction of the legs to decrease the compression of the femoral head against the acetabulum. As a result, the necrosis of the femoral head is reduced. During the period of treatment, the hip is able to move to prevent joint stiffness. The long-term successful results prove that this method has shortened the duration of treatment, giving good results, little pain and less complication.
