血清胱抑素C评价早期糖尿病肾损害的临床应用 被引量:2

The clinical application of Cystatin C for evaluating early damage in diabetic kidney disease

摘要 目的探讨胱抑素C(cystatin C,Cys C)对2型糖尿病患者早期肾损害的评价意义。方法对2010年1月~2011年12月我院53例2型糖尿病患者按照MDRD公式计算出的eGFR进行分组,分别测定各组患者的血肌酐(Scr)、Cys C及尿微量白蛋白(mAlb),分析Cys C与Scr、mAlb的异常表达率以及与eGFR的相关性。结果 GFR正常组,mAlb的异常表达率为83.33%,而GFR轻度降低组mAlb和Cys C的异常表达率为66.67%和100%。结论在2型糖尿病患者中,CysC和mAlb比Scr更敏感地反映了肾小球滤过率的变化,mAlb敏感性最高,其次为Cys C,可以早期预测糖尿病肾脏损害的发生。 Objective To explore the relationship of Cystatin C and the decrease of glomerular filtration rate in patients with type 2 diabetic mellitus. Methods The glomerular filtration rate of the 53 patients with type 2 diabetic mellitus was calculate by the MDRD formula. All the patients were from January 2010 to December 2011 in our hospital. Serum creatinine (Set) amt Cystatiri C were measured in all patients. Then we analyzed the relationships between GFR and Cystatin C, Set and Urine micro albumin (mAlb). Results The abnormal expression rate was 83.33% in patients with normal renal function, and were 66. 67% and 100% in patients with mild decreasing of GFR. Conclusion Cys C and mAlb were more sensitive than Scr to predict the ear- ly damage of diabetic kidney disease. The mAlb was the most sensitive data to predict the damage of renal function, the second one was Cys C.
