老年患者前列腺增生经尿道电切术后精神障碍分析及处理 被引量：2

摘要 目的探讨经尿道前列腺电切术(TURP)后精神障碍的相关因素及处理方法。方法回顾性分析本院自2006年1月至2011年12月TURP术985例,年龄59～90岁。合并冠心病54例,高血压173例,糖尿病109例,肾功能不全75例。烟酒嗜好者有328例。结果术后出现精神障碍者34例,60～70岁4例(0.4%),70～80岁19例(1.9%),80岁以上11例(1.1%)。70岁以上患者精神障碍发生率明显高于70岁以下(P<0.01)。根据病情程度予以心理安抚,安定或氯丙嗪镇静治疗,同时纠正低氧血症、水电解质紊乱,加强术后监护措施。结论高龄、术前基础疾病、术后应激、电解质紊乱、低氧血症是TURP术后并发精神障碍的高危因素。加强术前、术中及术后管理是预防及治疗该并发症的关键。
