摘要
目的:探讨火针配合活络固本汤治疗中风后偏瘫肢体功能障碍的治疗效果。方法:将114例该病患者随机分为火针疗法组(对照组1)38例,每2日火针相关穴位1次;益气活血疗法组(对照组2)32例,内服活络固本汤,每日2次;火针疗法配合益气活血疗法组(治疗组)44例,每隔日火针相关穴位1次,同时配合内服活络固本汤,每日2次;4周后观察各组患者的临床疗效。通过中医证候疗效评定标准及神经功能评分标准,比较3种疗法的差异,同时按照日常生活活动能力(ADL)评定标准和运动功能评分标准,得出相应的BI和Fugl-Meyer评分并加以对比。结果:4周后,按照中医证候疗效评定标准及神经功能评分标准,得出治疗组总有效率较对照组明显提高;且治疗组BI,Fugl-Meyer评分明显高于对照组,经统计学分析,3组评分差异均具有统计学意义(P<0.05)。结论:火针疗法配合活络固本汤可在短期内改善偏瘫患者功能障碍,二者具有协同效应,疗效确切,可较好地改善患者生存质量。
This study was aimed to investigate curative effect of fire needle with Huoluo Guben （HLGB, i.e., tendon-activating and body-strengthening） Decoction for relieving limb dysfunction of post-stroke hemiplegic patients. A total of 114 patients were randomly divided into 3 groups. There are 38 patients in the fire needle group （comparison group 1）. And acupuncture treatment was given every other day. There were 32 patients in the Yiqi Huoxue （YQHX, i.e., qi-benefiting and blood-activating） group （comparison group 2）. And the HLGB decoction was given twice a day. There were 44 patients in the fire needle with YQHX group （treatment group）. Acupuncture treatment was given every other day and HLGB decoction was given twice a day. Curative effects of patients in each treatment group were observed after 4 weeks. Differences among three groups were compared based on evaluation criteria of curative effect of traditional Chinese medicine （TCM） syndromes and nerve system functions. At the same time, grading and comparison on corresponding BI and Fugl-Meyer were given according to evaluation criteria of activities of daily living （ADL） and functional movement analysis （FMA）. The results showed that after 4 weeks treatment, the total curative effect rate of treatment group is obviously improved compared with comparison groups based on evaluation criteria of curative effect for TCM syndrome and nerve system functions. The grading on BI and Fugl-Meyer of treatment group is also much higher than comparison groups. There was statistical difference of grading among three groups based on statistical analysis （P 〈 0.05）. It was concluded that fire needle with HLGB decoction treatment can improve limb dysfunction of hemiplegic patients in short-term. The associated effect and curative effect are assured in the living quality improvement of patients.
