外周血胎盘生长因子水平可预测急性冠脉综合征患者的长期风险 被引量:2

摘要 目的研究急性冠脉综合征(ACS)患者急性期外周血PLGF水平对心脑血管事件再发的影响。方法采用前瞻性队列研究方法,纳入194例ACS患者,收集临床资料并检测急性期外周血PLGF水平。随访180天,首要终点包括死亡、心肌梗死、不稳定型心绞痛及卒中。采用Kaplan-Meier法及Cox回归分析PLGF对于首要终点风险的影响。结果 PLGF水平与高血压病史显著相关(P=0.040),与年龄、性别、糖尿病、高低密度脂蛋白血症、吸烟、肥胖无显著相关性(P>0.05)。Kaplan-Meier法分析提示高PLGF水平组首要终点发生率显著升高(P=0.002)。以年龄、性别、高血压、糖尿病、高低密度脂蛋白血症病史、吸烟及肥胖为协变量采用Cox回归分析提示基线PLGF升高,首要终点发生风险显著升高(PLGF最高组vs PLGF最低组HR=2.57,95%CI:1.09~6.05,P=0.031)。结论 ACS急性期外周血PLGF水平升高是心脑血管事件复发风险升高的独立危险因素。 Objective To investigate the influence of peripheral PLGF level on the recurrence of cardiovascular and cerebrovaseular events in acute coronary syndrome. Methods Totally 194 patients with acute coronary syndrome were enrolled. Clinic characteristics were collected, and peripheral PLGF in acute phase was examined. All patients were followed up for 180 days. Primary endpoints includ- ed death, cardiac infarction, unstable angina and stroke. Kaplan - Meier method and Cox regression were used to analyze the influence of PLGF to the risk of primary endpoints. Results PLGF level correlated with hypertension history(P = 0. 040) significantly, but had no relationship with age, sex, diabetes mellitus, hyperlipidaemia of low density lipoprotein, smoke or obesity(P 〉 0.05 ). Kaplan - Meier method suggested high PLGF level group had high incidence of primary endpoints (P =0. 002). Adjusted by age, sex, hypertension, dia- betes mellitus, hyperlipidaemia of low density lipoprotein, smoke and obesity, Cox regression suggested risk of primary endpoints in- creased with basic PLGF level( PLGF highest level group vs PLGF lowest group HR = 2.57,95 % CI 1.09 - 6.05, P = 0. 031 ). Conclu- sion Peripheral PLGF level in ACS is an independent risk factor of recurrence of cardiovascular and cerebrovascular events.
