摘要
目的应用定量组织速度成像(QTVI)技术评价单次血液透析对尿毒症患者左心房功能的影响。方法 30例既往无心血管病史的尿毒症患者,进行常规血液透析,分别于透析前、后1h行超声心动图检查。二维上测量左心房最大长径、横径并计算左心房最大容积;脉冲多普勒血流成像测量二尖瓣口血流峰值速度E、A、E/A;定量组织速度成像分别测量房间隔、左心房侧壁、前壁及下壁的心室收缩期心肌运动速度s峰、心室舒张早期心肌运动速度e峰、心室舒张晚期心肌运动速度a峰,并计算4个位点的平均值。结果尿毒症患者透析后与透析前相比,二尖瓣口血流频谱E峰降低(P<0.05),A峰差异无统计学意义(P>0.05),E/A下降(P<0.05);左房心肌运动速度s峰、e峰差异无统计学意义(P>0.05),a峰增高(P<0.05)。结论定量组织速度成像能客观地反映出透析对尿毒症患者左心房功能的影响。
Objective To evaluate the changes of the left atrial function in patients with uremia before and after hemodialysis by quantitative tissue velocity imaging(QTVI). Methods Thirty patients with uremia without overt heart disease, receiving regular hemodial- ysis, were studied by eehoeardiography within one hour before and after hemodialysis. The biggest volume of LA were measured by two - dimensional eehoeardiography. The peak E velocity ( E), the peak A velocity (A) and E/A were measured by doppler eehocardiography. The velocity of the left atrium in ventrieular systole, early ventricular diastole and late ventrieular diastole were measured on QTVI at left atrial of each walls ( anterior, inferior, lateral and septal) and the average of the four sites were calculated. Results Compared with before hemodialysis, E and E/A ratio decreased in patients with uremia after hemodialysis (P 〈 0.05 ) , while A was not significantly different( P 〉 0.05 ). The velocity of the left atrium were increased in ventricular systole and late ventricular diastole after hemodialysis ( P 〈 0.05 ) , while the velocity of the left atrium in early ventrieular diastole was not significantly different ( P 〉 0.05 ). Conclusion QTVI can evalu- ate the changes of the left atrial function in patients with uremia before and after hemodialysis more authentically.
