摘要
目的:探讨不同类型的侵袭性牙周炎(Aggressive Periodontitis,AgP)患者牙周基础治疗的方法及效果。方法:回顾性分析了2005.10-2011.10就诊我院的120例侵袭性牙周炎(AgP)患者的临床资料,其中广泛型侵袭性牙周炎(Generalized AggressivePeriodontitis,GAgP)56例,局限型侵袭性牙周炎(Localized Aggressive Periodontitis,LAgP)64例。本组所有患者均需进行牙周基础治疗,分别于治疗之前以及治疗后2、4、6个月对患者全口牙的探诊深度(Probing Depth,PD)、临床附着丧失(Clinical AttachmentLoss,CAL)、出血指数(Bleeding Index,BI)以及牙齿松动度(Tooth Mobility,TM)进行记录,组间差异以P<0.05表示具有统计学意义。结果:(1)GAgP与LAgP患者在治疗后的2、4、6个月的PD、CAL、BI以及TM较治疗前均有明显地改善,且具有显著的统计学差异(P<0.05);(2)GAgP患者的PD、CAL在治疗后的6个月与治疗后4个月比较,LAgP患者的PD、CAL在治疗后的4个月与治疗后的2个月比较其差异均存在统计学意义(P<0.05);(3)GAgP与LAgP患者第一恒磨牙治疗后的2、4、6个月的CAL较同期中切牙的改善情况更为显著(P<0.01)。结论:牙周基础治疗对GAgP与LAgP具有较好的临床治疗效果,上述两种类型的侵袭性牙周炎患者牙周基础治疗后的中期临床效果存在显著的差异性。
Objective： To investigate the clinical treatment and treatment effect for different types of aggressive periodontitis（AgP） patients.Methods： We retrospectively analyzed 120 cases of patients with aggressive periodontitis（AgP） who were admitted to our hospital during 2005.10-2011.10,and there were 56 cases of generalized aggressive periodontitis（GAgP） and 64 cases of localized aggressive periodontitis（LAgP）.All patients in this group should be subject to initial periodontal treatment.Before treatment and 2,4 and 6 months after treatment,the patient full mouth of teeth,probing depth（Probing Depth,PD）,clinical attachment loss（Clinical Attachment Loss,CAL）,bleeding index（Bleeding Index,BI）,and tooth mobility（Tooth Mobility,TM） were recorded.All patients accepted the clinical treatment of periodontal.And the difference between the groups was statistically significant（P 0.05）.Results：（1） The PD,CAL,the BI and TM of GagP were significantly improved after treating,and there was a significant difference（P 0.05） between before and after treating.（2） In the group of GAgP patients with PD,CAL,there were significantly differences between 6 months,4 months and 2 months（P〈0.05）.（3） CAL of Patients with GagP and LAgP 2,4,6 months after the first permanent molars treatment was improved more significantly than the incisors in the same period（P 0.01）.Conclusion： The periodontal therapy on the different types of aggressive periodontitis（GAgP and LAgP） showed good clinical results.And the mid-term clinical effects for the above two types of aggressive periodontitis patients were significantly different after periodontal treatment.
