摘要
目的 探讨肺癌骨转移的临床特点。方法 临床资料回顾性分析。结果 肺癌骨转移单发者占 19.1% ,多发者 80 .9%。肺癌骨转移患者中腺癌 5 8.5 % ,鳞癌 2 0 .2 % ,小细胞未分化癌 14 .9% ,大细胞癌 2 % ,腺鳞癌 2 % ,类癌1%。骨转移部位为肋骨 5 5 .3 % ,胸椎 46 .8% ,骨盆 40 .4% ,腰椎 31.9% ,其余为肩岬骨、胸骨、四肢骨等。腺癌以肋骨、胸椎、骨盆转移为主 ,鳞癌以肋骨、胸椎转移为主 ,小细胞肺癌以肋骨、骨盆、胸椎转移为主。在行X线、CT、MRI检查的病人中 ,骨转移以溶骨性破坏的占 86 % ,成骨性破坏 6 .9% ,混合性骨损害 6 .9%。 94例病人中 ,仅发现 1例有高血钙 ,34例有血清AKP增高。结论 肺癌骨转移以多发性、溶骨性骨破坏为特征。腺癌骨转移发生率最高 ,其次为鳞癌及小细胞肺癌。高钙血症与是否有骨转移无关。血清AKP值对肺癌骨转移诊断有参考价值。
Objective To research the clinical features of lung cancer with osseous metastases.Methods The clinical materials were retrospectively analysed.Results 80.9% patients had multiple osseous metastases and 19.1% had single metastasis.Out of 94 lung cancer patients with osseous metastases,58.5% was adenocarcinoma,20.2% was squamous carcinoma,14.9% was small cell lung cancer,2% was large cell carcinoma,2% was adeuosquamous carcinoma,1% was carcinoid carcinoma.The metastatic sites were ribs(55.3%),thoracic vertebrae(46.8%),pelvis(40.4%),lumber vertebtae(31.9%),scapulae,sternum and limbs etc.The most common metastatic sites were ribs,thoracic vertebrae and pelvisc for adenocarcinoma,ribs and thoracic vertebrae for squamous carcinoma,ribs、pelvis and thoracic vertebrae for small cell lung cancer.In patients who had been performed X ray、CT、MRI examination,86% showed osteolytic damage,6.9% showed osteoblastic damage,6.9% showed mixed damage.In 94 patients,only one had been found to have hypercalcemia,34 had elevated serum AKP level.Conclusions Osseous metastasis in lung cancer is characterized by multiple osteolytic damage.Adenocarcinoma has the highest rate of osseous metastasis.The next is squamous carcinoma and small cell lung cancer.Hypercalcemia is not related with osseous metsatasis.Elevated serum AKP level is helpful for the diagnosis of osseous metastases of lung cancer.
