摘要
目的:对上海交通大学医学院附属第六人民医院静脉用药调配中心(PIVAS)的不合理用药医嘱进行分析,以期为临床合理用药提供参考。方法:收集2010年1月-2012年12月该院PIVAS的电子医嘱共计444万余条,对医嘱的用药合理性进行分析。结果:该院PIVAS涉及不合理用药的医嘱共计2 708条,占0.61‰,主要表现为配伍禁忌、溶媒选择不当和药品用法用量不当等。结论:PIVAS的药师审方能促进临床合理用药。
Objective： To analyze the irrational use of drugs in pharmacy intravenous admixture service（PIVAS） of Sixth People＇s Hospital affiliated to School of Medicine,Shanghai Jiaotong University,so as to provide reference for rational use of drugs.Methods： More than 4.44 millions electronic prescriptions in PIVAS of the hospital from January 2010 to December 2012 were collected and their rationality was analyzed.Results： Totally,2 708（0.61‰） prescriptions were involved in the irrational use of drugs,such as incompatibility,improper selection of solution,wrong dosage and medication,etc.Conclusion： Pharmacists in PIVAS could enhance rational use of drugs by checking prescriptions carefully.
