摘要
目的探讨64层螺旋CT血管成像(CTA)对颅内动脉瘤的临床诊断价值。方法对本院收治、疑有颅内动脉瘤的71例患者进行DSA造影和64层螺旋CTA检查,比较两种检查方法的成像效果;并以DSA为标准,分析CTA检查颅内动脉瘤的敏感性、特异性、阳性预测值及阴性预测值等诊断学评价指标。结果成像效果:在动脉瘤的形态、大小、位置方面,两种方法均获得清晰图像,CTA除1例漏诊外,检查结果均与DSA相符;但对于病变血管壁内血栓、钙化状态及病变位与周围组织、血管的关系,CTA显示效果优于DSA。诊断学评价:CTA对颅内动脉瘤诊断的敏感性、特异性、阳性预测值及阴性预测值分别为98.7%、100%、100%、90%,均达到较高水平。结论 CTA对颅内动脉瘤的诊断在敏感性、特异性、阳性预测值及阴性预测值上可取得与DSA相近的结果,且在钙化、血栓,及病变区域与周围组织、血管的关系方面较DSA影响更清晰。
Objective To investigate the clinical diagnostic value of 64-slice spiral CT angiography （CTA） on intracranial aneurysms. Methods Seventy one patients suspected with intracranial aneurysms in our hospital were examined by DSA imaging and 64-slice spiral CTA, and the imaging results of the two methods were compared with each other; The diagnostic evaluation of the CTA examination for intracranial aneurysm was analyzed through its sensitivity, specificity, positive predictive value and negative predictive value with DSA as the standard. Results Imaging results： The two methods were both got clear image in size, location and morphology of the aneurysm, and the examination results of CTA were all consistent with the DSA in addition to 1 case of missed diagnosis; While CTA display effect was better than DSA in aspects of thrombus within the vascular wall lesions, calcification state, the relationship of the surrounding tissue and blood vessel with its lesions. Diagnostic evaluation： The sensitivity, specificity, positive predictive value and negative predictive value of CTA diagnosis for intracranial aneurysms were 98.7%, 100%, 100%, 90%, which had reached a high level. Conclusion CTA diagnosis can obtain similar results with DSA for intracranial aneurysms in the sensitivity, specificity, positive predictive value, and negative predictive value. And the impacts in calcification, thrombus, lesion area, and the vascular relations with surrounding tissue are even clearer than DSA.
