不同检测部位彩色多普勒与B-Flow血流显像对2型糖尿病下肢血管内皮功能受损的影响 被引量:1

摘要 目的探索彩色多普勒与B-Flow血流显像对2型糖尿病下肢血管内皮功能检测的最佳部位。方法选择2型糖尿病无并发症患者(Ⅰ组)、有并发症患者(Ⅱ组)及正常对照组各256例,受损依据探测部位(胫前左侧右侧、胫后左侧右侧)及处理方式(基础内径、反应性充血及硝酸甘油诱发后的内径)的不同,将应变量分为12组,分别进行组间差别及影响因素多元线性回归分析。结果组间比较分析显示基础内径、反应性充血后内径、硝酸甘油诱发后内径比较,3组之间差异均有统计学意义(P<0.01)。多因素分析显示基础内径胫前右侧,反应性充血后内径胫前左侧,硝酸甘油诱发后内径胫前右侧、胫后左侧引入变量均较其他类别为多。结论综合组间比较和多因素分析结果,建议临床采用B-Flow及多普勒超声进行血管内皮功能受损测定及有关研究时,最佳部位推荐胫前动脉左或右。 Objective To explore the best part of the color Doppler and B-Flow flow imaging on type 2 diabetic lower extremity vascular endothelial function detection. Methods Type 2 diabetes patients without complications ( I group), patients with complications (11 group) and normal control group were selected, each group contained 256 cases. Based on the detection part of the right side (the left anterior tibial, posterior tibial, left the right side) and treatment (base diameter, reactive hyperemia and nitroglycerin-induced diameter) for the impaired, the variables were divided into 12 groups, and were analyzed with difference between groups and influencing factors of multiple linear regression. Results The difference between groups analysis showed that there were significant differences among the three groups of basis of the inner diameter, inner diameter of the anterior tibial reactive hyperemiat, inside diameter after nitroglycerin-in- duced anterior tibial (P 〈 0.01). The multiple factors analysis showed that variables in the basis of the inner diameter of the anterior tibial right side, the inner diameter of the anterior tibial reactive hyperemia the left, inside diameter after nitroglycerin-induced anterior tibial right side, the left side of the posterior tibial were more than other types. Conclusion For the comparison between groups and multiple factors analysis results, we suggest that when taking B- Flow and color Doppler to detect the type 2 diabetic lower extremity vascular endothelial function impaired studies, the best parts recommend the left or right of the anterior tibial artery.
