摘要
目的观察马来酸依那普利叶酸片对伴有同型半胱氨酸(Hcy)升高的高血压患者的疗效。方法选择230例伴有血浆同型半胱氨酸升高的原发性高血压患者,随机分为观察组(马来酸依那普利叶酸片10mg+0.8mg)和对照组(马来酸依那普利10mg),总观察治疗时间为36周。评价指标有:血压水平、血浆Hcy、尿白蛋白/肌酐比值(A/C)、颈动脉内膜厚度(IMT)。结果治疗前观察组与对照组在血压水平、Hcy、尿A/C、IMT差异无统计学意义。用药治疗36周后观察组与对照组在收缩压(SBP)与舒张压(DBP)水平的控制上差异有统计学意义(P<0.05);对于血浆Hcy的控制上观察组与对照组差异有统计学意义(P<0.01);在尿A/C比值水平的控制上观察组与对照组差异有统计学意义(P<0.01);对于IMT的控制上观察组与对照组差异有统计学意义(P<0.01)。结论马来酸依那普利叶酸片对于伴有血浆Hcy水平升高的高血压患者(H型高血压)的血压水平、血浆Hcy、尿A/C比值、IMT均有明显的改善,延缓了动脉粥样硬化的进展,保护了靶器官的功能。
Objective To observe the therapeutic effect of malay acid enalaprilat folic acid tablets in hyper- tensive patients accompanied by elevated homocysteine(Hcy). Methods Total 230 patients with elevated plasma homocysteine in essential hypertension patients were randomly divided into experimental group (ma- leic acid enalaprilat folic acid tablets 10 mg + 0.8 mg) and control group (maleic acid enalaprilat 10 mg), all subjects were followed- up 36 weeks. Evaluation indexes included blood pressure, plasma homocysteine levels, the urinary albumin/creatinine ratio (A/C), carotid intimal thickness (IMT). Results Some inde- xes such as blood pressure, plasma homocysteine levels, the urinary albumin/creatinine ratio, carotid ar- tery intima thickness in experimental group and control group before treatment were no statistical differ- ence. On the control level of systolic blood pressure (SBP) and diastolic blood pressure (DBP) was statisti- cally difference after 36 weeks drug treatment in experimental group and control group(P d0.05) ; the lev- el of plasma homocysteine in experimental group was lower than control group(P d0.01). Urinary albu- min/creatinine ratio of trial group and control group was statistically difference (P d0.05), and carotid ar- tery intima thickness of experimental group and control group was also statistically difference (P d0.01). Conclusions For the hypertensive patients accompanied by elevated plasma homocysteine, maleic acid enal- aprilat folic acid tablets can obvious decreased the level of blood pressure and blood plasma homocysteine, optimize the urinary albumin/creatinine ratio, improve the thickness of the carotid artery intima, delay the progression of atherosclerosis, protect the function of target organs.
