摘要
目的比较腹腔镜结肠癌根治术与开腹结肠癌根治术治疗结肠癌的远期疗效。方法采用前瞻性随机对照研究,将收治的182例结肠癌患者随机分为腹腔镜手术组(LS组)与开腹手术组(OS组),每组91例。分别采用腹腔镜结肠癌根治术及开腹结肠癌根治术治疗,随访观察5年,比较两组患者局部转移复发情况及远期生存情况。结果 LS组患者转移复发率、3年生存率、5年生存率、无瘤生存时间和总生存时间分别为17.6%、83.5%、28.6%、(23.4±4.8)个月和(33.2±7.3)个月,OS组患者分别为18.7%、87.9%、30.7%、(25.1±5.1)个月和(34.7±6.6)个月。两组患者的转移复发率、3年生存率、5年生存率、无瘤生存时间和总生存期比较,差异均无统计学意义(P>0.05)。结论腹腔镜结肠癌根治术能够达到与开腹结肠癌根治术相同的远期疗效,且创伤小、愈合快、并发症少,值得临床推广应用。
Objective To compare the long-term efficacy of laparoscopic and open radical resection of colon cancer. Methods A prospective randomized controlled study was adopted. 182 cases of colon cancer treated in our hospital were randomly divided into laparoscopic surgery group （ LS group） and open surgery group （OS group）, and they were given laparoscopic radical resection of colon cancer and open rad- ical resection of colon cancer, the 5-year follow-up study was conducted to compare local transfer of recur- rence, and long-term survival between the two groups. Results metastatic recurrence rate, 3-year surviv- al, 5-year survival, disease-free survival and overall survival of LS group were 17.6% , 83.5% , 28.6%, （23.4 ±4. 8） months and （33.2 ±7.3） months, while those of OS group were 18.7% , 87.9%, 30.7%, （25.1±5.1） months and （34. 7 ±6. 6） months; there were no significant difference between the LS group and OS group in metastatic recurrence rate, 3-year survival rate, 5-year survival rate, free survival and o- verall survival （P 〉 0. 05）. Conclusions Laparoscopic radical resection of colon cancer can achieve the same long-term effect with the laparotomy colon cancer surgery, and has less trauma, faster healing, fewer complications, which is worthy of clinical application.
