摘要 目的 探讨在胸部CT图像上应用1997 AJCC—UICC胸内淋巴结分组的方法。方法 10例患者(淋巴瘤5例,结节病2例,转移性淋巴肿3例)均有胸部CT扫描。所见的胸内淋巴结用1997 AJCC-UICC方法进行分组。结果 应用1997 AJCC-UICC淋巴结分组方法在胸部上划分8个层区。Ⅰ第6颈椎上缘至肺尖包括锁骨上和斜角肌组;Ⅱ肺尖至无名静脉横行段包括IR/L和3A/3P组;Ⅲ无名静脉横行段至主动脉弓上缘包括2R/L和3A/3P组;Ⅳ主动脉弓上缘至主肺动脉窗包括4R/L、5和6组;Ⅴ主肺动脉窗至左肺动脉包括4R/L、5、6和10R/L组;Ⅵ左肺动脉至隆突包括4R/L、5、6和10R/L组;Ⅶ隆突下3cm以内包括7和10R/L组;Ⅷ隆突下3cm至膈上包括8R/L和9R/L组。在肺尖至膈上各层面肺内为11—14R/L组。结论 8个层区的划分,使1997 AJCC-UICC淋巴结分组方法在胸部中的应用更简便、实用。 Purpose To assess the application of the intrathoracic lymph nodes classification advocated by 1997 AJCC - UICC on CT scan. Methods Ten patients suffered from lymphoma (5 cases) ,sarcoidosis(2 cases) and metastatic lymphadenopathy(3 cases)were examinated with CT. The intrathoracic lymph nodes were grouped according to the AJCC - UICC nodal classification. Results Eight areas on the CT scan were divided to display the 14 groups of lymph nodes: I between the upper margin of the sixth cervical vertebra and the pulmonary apex, including groups 1R/L,3A/ 3P,Ⅲ between the upper rim of the left innominate vein and the upper of the aortic arch, including groups 2R/L, 3A/3P, Ⅳ between the upper margin of the aortic arch and aortopulmonary window, including groups 4R/L, 5,6, V between the aortopulmonary window and left pulmonary artery, including groups 4R/L,5,6,10R/L, Ⅵ between the left pulmonary artery and carina,including groups 4R/L,5,6,10R/L, Ⅶ within 3 cm below cari-na, including groups 7.10R/L, Ⅷ between 3 cm from subcarina and the top of diaphragm, including groups 8R/L,9R/L and intrapulmonary area, between the pulmonary apex and the top of diaphragm, including groups 11R/L - 14R/L. Conclusion The dividing of thorax into eight areas on CT scan makes the application of 1997 AJCC - UICC lymph nodes classification easier and applicable.
