HDCT能谱技术提高肝硬化门静脉血管成像图像质量的价值研究 被引量:16

摘要 目的探讨能谱技术在提高肝硬化患者CT门静脉血管成像质量方面的价值。方法 45例肝硬化患者,采用的能谱扫描模式行动脉期门脉期扫描后,运用能谱分析软件获得门静脉期最佳对比信噪比(CNR)单能图像和传统混合能量图像(QC图像),并对门静脉及侧枝血管进行容积再现(volume rendering,VR)、最大密度投影(maximum inten-sity projection,MIP)重建。采用双盲法对重建质量进行评估。对二组图像的评分数据行两独立样本T检验分析。结果最佳CNR单能图像的VR、MIP重建图像对门静脉与侧枝血管及肝内分支的显示级别均优于混合能量图像,差异有显著统计学意义(P<0.01)。结论最佳CNR单能图像能明显提高肝硬化CTPV门静脉与侧枝血管及肝内分支显示的图像质量。 Objective To investigate the method of improving the image quality of CT portal venography of patients with cirrhosis using spectral CT imaging. Methods 45 patients with liver cirrhosis underwent spectral CT imaging with GSI mode during the arterial and portal phase. The spectrum analysis was used to select the monochromatic images for obtai- ning the best contrast-to-noise ratio (CNR) for main portal veins, as well as the conventional polychromatic images. Image quality of MIP and VR reconstruction of portal venography, collateral vessels and the intrahepatic branches was evaluated in a double-blind manner by two radiologists. Two independent samples T-test analysis was used to deal with the data. Re- sults At the best energy level, the image quality of VR and MIP was higher than that of polychromatic images, with a significantly different ( P 〈0.01). Conclusion Monochromatic images at best keV for CT Portal Venography could im- prove the overall image quality of portal venography, collateral vessels and the intrahepatic branches.
