超声观察盆底器官脱垂女性盆底器官运动及角量参数变化 被引量:17

Pelvic floor motion and angular parameters variation in pelvic organ prolapse women using ultrasound imaging

摘要 目的应用经会阴二维超声观察盆底器官脱垂(POP)女性盆底器官运动及角量参数变化。方法选取POP患者50例(POP组)和非POP患者50例(对照组),使用经会阴二维超声观察其静息期、缩肛期和张力期盆底器官情况,获取不同时期尿道倾斜角(UIA)和肛直角(ARA)等角量参数。结果 POP组与对照组盆底器官运动偏转方向一致。POP组中11例患者静息期近段尿道向背侧倾斜(11/50),张力期有39例(39/50);对照组中3例患者静息期近段尿道向背侧倾斜(3/50),张力期有16例(16/50)。POP组各期UIA均小于对照组(P﹤0.05),缩肛期UIA偏转幅度小于对照组(P﹤0.001),张力期前腔脱垂患者UIA偏转幅度大于对照组(P﹤0.05)。POP组各期ARA及其变化幅度与对照组比较差异无统计学意义,但张力期后腔脱垂患者ARA变化幅度大于对照组(P﹤0.05)。结论经会阴二维超声能动态观察POP运动异常,间接反映盆底支持组织功能状态,为POP的诊断及盆底功能评价提供可靠信息。 Objective To investigate the pelvic organs dynamical motion and evaluate the angular parameters variation in pelvic organ prolapse women with transperineal two-dimensional ultrasound. Methods Transperineal two-dimensional ultrasound was performed on fifty female patients with POP(POP group) and fifty female patients without POP(control group).The real -time movement of pelvic organs was investigated and the angular parameters were detected when at rest, on anal sphincter contraction and on maximal Valsalva maneuver,respectively. The urethral inclined angle( UIA )and the anorectal angle (ARA) of different time were detected and compared. Results The pelvic organs motion in POP group was in accordance with that in control group. 11 patients in resting stage(11/50), 39 patients on Valsalva period(39/50) of POP group and 3 patients in resting stage(3/50), 16 patients in maximum Valsalva period (16/50) in control group whose proximal urethras were detected tilting to the dorsal. UIA of all stage in POP group was less than that of the control group(P 〈 0.05). The deflection amplitude of UIA contraction period in POP group was less than that of the control group (P 〈 0.001 ), the deflection amplitude of front chamber prolapse was bigger than that of the control group in Valsalva period(P 〈 0.05). No significant difference was found for the ARA and its amplitude of variation in all stages between in POP group and the control group, while the variation amplitude of ARA of posterior compartment prolapse was beyond that of the control group in Valsalva period (P 〈 0.05). Conclusion Transperineal two-dimensional ultrasound can demonstrate the pelvic floor organs motion clearly and dynamically. It is potentially able to be used in the diagnosis of POP and evaluate the function of pelvic floor in POP women.
