摘要
[目的]探讨膝关节屈曲畸形与腰椎和骨盆矢状位对线的相关性研究。[方法]回顾分析2009年1月2012年8月腰椎退行性疾病合并膝关节屈曲挛缩畸形的127例患者的临床资料,所有患者均行腰椎及膝关节X线片,使用PACS影像传输系统中的电脑软件测量以下角度:腰椎前凸角度(LL),骨盆投射角度(PI),骨盆倾角(PT),骶骨倾角(SS),膝关节屈曲角度(KF)。根据膝关节屈曲角度分组,比较各组测量参数及腰椎Oswestry评分(ODI)。[结果]KF与LL、PI及SS存在相关性(r=0.184,P﹤0.05;r=0.303,P﹤0.05;r=0.254,P﹤0.05),与PT无明显相关性(P=0.79)。LL与SS及PI存在相关性(r=0.768,P﹤0.01;r=0.550,P﹤0.01)。膝关节屈曲角度越大,腰椎前凸越明显(P﹤0.01),腰椎ODI评分升高。[结论]膝关节屈曲畸形的程度与腰椎和骨盆矢状位对线有相关性,与腰椎退行性疾病的临床症状呈正相关。
[Objective] To study the correlation between knee contracture deformity and lumbar- pelvis sagittal align- ment. [ Methods] The clinical data of 127 patients suffering from lumbar degenerative diseases and knee contracture deformity concurrently from January 2009 to August 2012 were retrospectively analyzed. All patients underwent radiological examination of lumbar spine and knee in standing position. The angles were measured by the computer software in PACS image transmission sys- tem: lumbar lordosis angle ( LL), pelvic incidence (PI), pelvic tilt (PT), sacral slope ( SS), knee flexion angle (KF) ~ According to the knee flexion angle, these patients were divided into three groups. Correlation analysis between the different measured parameters and lumbar Oswestry disability index (ODI) was performed. [ Results] The knee flexion angle correlated with lumbar lordosis angle and pelvic incidence angle and sacral inclination angle ( r = 0. 184, P 〈 0. 05 ; r = 0. 303, P 〈 0. 05 ; r = 0. 254, P 〈 0. 05), no correlation with pelvic tilt angle was found. Lumbar lordosis angle correlated with sacral inclination angle and pelvic incidence angle ( r = 0. 768, P 〈 0. 01 ; r = 0. 550, P 〈 0. 01 ) . Lumbar lordosis angle and Oswestry disability index had increased when the knee was flexed. [ Conclusion ] Knee contracture deformity correlates with the lumbar and pelvic sagittal alignment and positively correlates with the clinical symptoms of lumbar degenerative disease.
