摘要
目的探讨横裁带蒂包皮岛状皮管尿道成形术(Duckett)在中、重度尿道下裂修复中的应用。方法总结2005年1月至2012年10月应用Duckett及Duckett联合Duplay术式的380例尿道下裂患儿临床资料,年龄1～11岁,平均4.3岁。结果本组患儿均获得随访,随访时间2～36个月,平均14个月。一次手术成功329例,成功率为86.58%。术后并发症51例(13.42%),其中尿道瘘19例(5.00%),尿道憩室16例(4.21%),尿道狭窄11例(2.90%),尿道瘘合并憩室5例(1.31%)。发生尿道瘘及尿道憩室的40例患儿均在尿道成形术后6～12个月分别行尿道瘘修补术及尿道憩室切除术,其中33例经1次修复手术痊愈,7例经2次修复手术痊愈。术后11例尿道狭窄患儿中,7例经尿道扩张后痊愈,4例行尿道切开术。结论横裁带蒂包皮岛状皮管尿道成形术治疗中、重度尿道下裂效果比较满意。
Objective To study the clinical effect of tubularized transverse preputial island flapurethroplasty （Duckett） for the repair of hypospadias. Methods We retrospectively studied 380 cases of hypospadias treated by Duckett＇s or Duckett and Duplay＇s procedure performed between January 2005 and Octorber 2012. The mean age of the patients was 4.3 years （range from 1 to 11 years）. Results We followed up the patients for 2~36 months. 329 cases were cured （86.58%） by Duckett procedure. Fifty-one patients（13.42%） had postoperative complications, including fistula in 19, stenosis in 11, diverticulum in 16, fistula and diverticulum in 5. Among the cases of fistula and diverticulum, 33 cases were repaired successfully by one additional surgery, and another 7 cases required two additional procedures. Seven cases of stenosis were cured by dilatation, and the other 4 cases of stenosis required urethrotomy for treatment. Conclusion Tubularized transverse preputial island flap urethroplasly provides satisfactory effect for the treatment of severe hypospadias.
