摘要
目的从分子病理学水平探讨 HCV 基因型与 HCC 发生的关系。方法采用逆转录原位 PCR 法对95例 HCC 及其非癌组织中1b 型和2a 型 HCV 进行检测,同时采用 Okamoto 法对 HCC 组织 RNA 抽提物中 HCV 基因型加以验证。结果 HCC 中 HCV 阳性率为44%(42/95),检出3种 HCV 基因型,包括1b(13例,31%)、2a(23例,55%)、混合型(1b+2a,2例,5%)和3a(4例,10%),1b 型检出率(14%)与2a 型者(24%)无显著性差异(X^2=3.427,P>0.05),两型与 HCC 主要病理学参数的相关性相似,2a 型阳性细胞在癌和非癌区的分布相似,而1b 型在癌组织多呈局灶分布,在非癌区多呈局灶/弥漫分布,HCC 中1b 型 HCV 的核-胞质型/核型阳性信号较2a 型者明显多见(X^2=4.073,P<0.05),去除逆转录并用RNase A 处理后,1b 型和2a 型 HCV 仍分别有5例和9例可见阳性信号。结论我国两种常见的 HCV 基因型与 HCC 发生的相关性相似,HCC 中 HCV 相关 DNA 及其与宿主基因组整合的可能性不能除外。
AIM To study the relationship between hepatitis C virus (HCV)genotypes and development of hepatocellular carcinoma(HCC)at molecular pathologic level, METHODS Reverse transcription in situ polymerase chain reaction(RT-IS PCR)method was performed to detect genotypes 1b and 2a of HCV in HCC tissue and noncancerous area of 86 cases.HCV genotypes were simultaneously checked by Okamotos method using RNA extracts from HCC tissues, RESULTS The positivity of HCV in HCC tissue was 44% (42/95).Three HCV genotypes were detected,including 1b,2a,mixed type(1b+2a)and 3a in 13(31%),23 (55%),2 (5%) and 4(10%) cases respectively.There was no significant difference between positivities of genotypes 1b(14%)and 2a(24%)(X^2-3.427,P>0.05). And two genotypes showed similar correlation with main pathologic parameters of HCC.The distribution of genotype 2a-positive cells was similar in both cancerous and noncancerous areas.Cells with genotype 1b were prone to be focal in cancerous area and focal/diffuse in noncancerous area.The positive nucleocytoplasmic/ nuclear signals of genotype 1b in HCC were seen more frequently than that of genotype 2a(X^2=4.073,P<0.05). With omission of reverse transcription and with pretreatment of RNase A,the positive signal of genotypes 1b and 2a were still observed in 5 and 9 HCC cases respectively. CONCLUSION Two common HCV genotypes(1b and 2a) are similarly associated with HCC in China,The possibility of presence of HCV-related DNA and its integration into host genome could not be excluded,
