摘要
目的:系统评价通心络胶囊治疗偏头痛的有效性和安全性。方法:计算机检索MEDLINE、EMbase、Cochrane图书馆、中国生物医学文献数据库、中国期刊全文数据库、中文科技期刊数据库、中文生物医学期刊数据库检索,并追索纳入文献的参考文献和相关文献,检索截止时间2010年2月。纳入通心络胶囊治疗偏头痛的随机对照试验,利用Cochrane手册5.0推荐的"偏倚风险评估"工具对纳入研究进行质量评价。对同质研究采用RevMan5.0软件进行Met a分析,对非同质资料进行描述性分析。结果:共纳入10篇文献,均为随机对照试验,共770例患者。根据测量指标、干预措施进行分析,结果显示,10个研究报道了有效率,Met a提示通心络胶囊治疗偏头痛与西比灵总效率[RR=1.18,95%CI(1.05,1.33),P=0.007],差异有统计学意义。3个研究比较了偏头痛发作次数,2个研究报道了偏头痛发作持续时间。1个研究报告了上腹不适,但症状轻微,可自行缓解。结论:Met a分析显示通心络胶囊可改善偏头痛患者症状,疗效优于西比灵,但因纳入文献质量较差,病例数少,并且尚无长期疗效的报道,因此其结论尚需进一步验证。
Objective： To evaluate the safety and effectiveness of TongXinLuo capsule in the treatment of mi- graine. Method： We searched MEDLINE, EMbase, the Cochrane library, China Biological Medicine Database, Chi- nese Academic Journal, Chinese Scientific Journals Database and Chinese Biomedical Database Retrieval with com- puter, searched for the reference and associated literature of included literature by February, 2010. Randomized con- trolled experiments about TongXinLuo capsule were included and quality of the included studies was assessed with bias assessment tool recommended in the Cochrane Handbook 5.0. Meta-analysis of homogeneous studies was per- formed with RevMan5.0 software and descriptive analysis of non-homogeneous data was studied. Result： Ten pieces of articles were about randomized controlled trials, which contained 770 patients. The results of measure and inter- vention analysis showed there were ten studies reporting effective rate. Meta analysis suggested that the comparison between TongXinLuo capsule and sibelium in treating migraine was [-RR=I.18, 95%CI （1.05, 1.33）, P=0.007] and the difference was significant. There was three studies comparing the onset times of migraine, two studies reporting the duration of migraine, one study telling epigastric discomfort, but the symptoms were mild, most were minor and could relieved by themselves. Conclusion： Meta analysis shows that TongXinLuo capsule can improve migraine symptoms, and the effect are better than Sibelium. But some literatures of poor quality and small sample size are in- cluded, and no long-term efficacy reports, so its conclusions need fiarther validation.
