摘要
目的:探讨超声乳化白内障吸出术和小切口非超声乳化白内障囊外摘除术在高龄白内障患者中的应用疗效。方法:将120例高龄白内障患者随机分为A、B两组,每组60例。A组采用超声乳化白内障吸出术治疗,B组采用小切口非超声乳化囊外摘除术治疗,对比两组术后视力、术后散光度数、术中与术后并发症发生率。结果:(1)术后3天后B组的视力均要高于A组,差异有统计学意义(P<0.01),两组术后1个月及3个月视力对比,A组的视力均要高于B组,差异无统计学意义(P>0.05),且均与术前视力相比存在显著的统计学差异(P<0.01);(2)A、B两组患者在术后1个月及3个月散光度对比,均无统计学差异(P>0.05),且两组术后1个月与术前相比差异具有统计学意义(P<0.05),术后3个月恢复至术前水平(P>0.05);(3)A、B两组术中与术后并发症的发生率分别为18.33%(11/60),23.33%(14/60),二者差异具有统计学意义(P<0.05)。结论:小切口非超声乳化囊外摘除术治疗高龄白内障患者,其效果与超声乳化吸出术接近,且手术费用低、操作简便、术中及术后并发症发生率小,适合于广泛地应用。
Objective： To compare effect of ultrasonic emulsification aspiration of cataract and non- ultrasonic emulsification extracapsular cataract extraction with small incision on older patients with cata- ract Methods： A total of 120 elder cases with cataract were randomly divided into two groups by half. Pa- tients in A group had ultrasonic emulsification aspiration of cataract, while group B had non-ultrasonic e- mulsification extracapsular cataract extraction with small incision. Visual acuity, postoperative astigma- tism degree, intraoperative and postoperative complications were observed and compared. Results： Threedays after surgery, vision of group B was significantly better than group A （P-~0.01）, 1 month and 3 months after surgery, the vision was better in group B, but the difference was not significant （P〈0.05）. Vision was significantly improved in two groups after treatment （P〈0. 01）. There was no significant difference in astigmatism 1 month and 3 months after surgery between two groups （P〈0.05）, it recov- ered to the level before surgery 3 months later after surgery. Complications incidence was 18.33% （11＇/ 60）, 23.33% （14/60） in two groups, with significant difference between two groups （P〈0.05）. Conclu- sion： Non-ultrasonic emulsification extracapsular cataract extraction with small incision is as effective as ultrasonic emulsification aspiration of cataract. But it is of low cost, easy process, and little complication incidence, and it is valuable in clinical application.
