摘要
目的探讨肿瘤坏死因子-α(TNF-α)、瘦素、可溶性血管内皮细胞黏附分子1(sVCAM-1)水平在2型糖尿病(T2DM)患者血清中的变化及其与T2DM心脑血管并发症的相关性。方法选择健康体检者30例为对照组,T2DM患者34例为糖尿病组,T22DM合并心脑血管病变者41例为并发症组。采用双抗体夹心法酶联免疫吸附试验检测各组患者血清TNF-α、瘦素和sVCAM-1水平;采用全自动血生化分析仪和化学发光法分别检测各组患者空腹血清葡萄糖(FPG)和空腹血清胰岛素(FINS)水平,采用稳态模型评估法(HOMA)计算胰岛素抵抗指数(HOMA-IR)。结果糖尿病组、并发症组血清TNF-α、瘦素和sVCAM-1水平及HOMA-IR均显著高于对照组,差异有统计学意义(P<0.01);糖尿病组和并发症组间各指标比较差异均有统计学意义(P<0.01)。糖尿病组、并发症组TNF-α、瘦素和sVCAM-1水平与HOMA-IR成正相关(r=0.452、0.489、0.442、0.476、0.569、0.572,P<0.05),并发症组TNF-α、瘦素、sVCAM-1与HOMA-IR的相关系数均高于糖尿病组TNF-α、瘦素、sVCAM-1与HOMA-IR的相关系数。结论 TNF-α、瘦素和sVCAM-1是T2DM及其心脑血管并发症发生发展的高危因素;TNF-α、瘦素和sVCAM-1与T2DM胰岛素抵抗明显相关。
Objective To investigate the changes of tumor necrosis factor-α(TNF-α),leptin,soluble vascular cell adhesion molecule-1(sVCAM-1)levels in blood serum of patients with type 2 diabetes mellitus(T2DM) and the relations between the three index and cardio-cerebrovascular complications.Methods Thirty healthy people who had medical examination were chosen as control group,34 patients with T2DM were chosen as diabetes group and 41 patients with T2DM and cardio-cerebrovascular diseases were chosen as complication group.A double antibody sandwich enzyme-linked immunosorbent assay was used to detect the levels of TNF-α,leptin,sVCAM-1 in blood serum of the subjects.Automatic blood biochemical analyzer and chemiluminescence method were used respectively to detect the contents of fasting serum glucose(FPG)and fasting serum insulin(FINS)of the subjects,and insulin resistance(IR)index was estimated by homeostasis model assessment(HOMA).Results The levels of TNF-α,leptin,sVCAM-1 and HOMA-IR of the diabetes group and complication group were significantly higher than those in control group,the differences were statistically significant(P〈0.01).There were significant differences in above indexes between diabetes group and complication group(P〈0.01).The levels of TNF-α,leptin and sVCAM-1 had significant positive correlation with HOMA-IR in diabetes group and complication group(r=0.452,0.489,0.442,0.476,0.569,0.572,P〈0.05),and all the correlation coefficients in complication group were higher than those in diabetes group.Conclusion TNF-α,leptin and sVCAM-1 are high risk factors for T2DM and its cardiovascular and cerebrovascular complications development,and the 3 indexes are significantly correlated with IR of T2DM.
