摘要
目的探讨基底节区高血压脑出血经侧裂岛叶入路的显微手术治疗的手术技巧及效果。方法回顾分析我科自2010年1月~2013年1月共36例患者的手术治疗情况及患者术后恢复效果。结果恢复良好者均留有出血对侧肢体不同程度的偏瘫,死亡1例,按GOS预后评分,4分(轻度残疾)23例,3分(重度残疾)8例,分(植物生存)4例,1分(死亡)1例。结论经侧裂岛叶入路基底节区高血压脑出血的显微手术治疗,可减少术中的损伤,同时要求术者有良好的显微解剖知识及熟练的显微操作技术。
Objective To evaluate the application of microsurgery treatment of hyperten- sive cerebral hemorrhage in the basal ganglia region through transsylvian approach. Methods 36 cases of hypertensive cerebral hemorrhage in the basal ganglia region from January, 2010 to Janu- ary, 2013 were analyzed retrospectively. Results Hemiplegia was find in all patients, we classi- fied therapeutic effect by Glasgow Outcome Scale (GOS) scores. 23 patients were scored 4(slight disability), 8 patients were 3(severe disability), 4 patients were 2(vegetative state) and one pa- tient was 1(died). Conclusion Microsurgery through transsylvian approach was a minimally-in- vasive and safe treatment of hypertensive cerebral hemorrhage in the basal ganglia region, mean- while abundant microanatomy knowledge and perfect micromanipulative technique were necessary to the surgeons.
